Job summary

We are looking to recruit an experienced registered mental health nurse who is enthusiastic, compassionate and caring to join the Hammersmith and Fulham Health Based Place of Safety team as a Clinical Team Leader.

The HBPoS in H&F provides a place of safety for both detained under S135 and S136 of the Mental Health Act and patients that have been diverted from Charing X Emergency Department.

The service provides a safe place for those awaiting to be assessed by the HBPoS team, S12 approved doctors and AMHP.

The post holder will provide care, support and assessments of all clients who enter the HBPoS.

The post holder will be given clinical supervision by qualified staff and management responsibility is with the HBPoS Team Manager.

To meet with police and service user to complete assessment outside of HBPoS in the company of an unqualified clinician.

To accept S136/S135 paperwork from police/AMHP and ensure all information is accurately correct including time of arrival to the HBPoS.

Main duties of the job

You will be able to demonstrate excellent communication skills while providing first line advice, support, and information to a wide range of clients.

The post holder will provide support when someone, both known to services and unknown comes to the unit. You will need to have important assessment skills, a knowledge of the legal framework of mental health law and an understanding of mental health diagnosis, as well as an interest in psychosocial interventions.

To provide basic nursing care to clients including physical assessments, monitoring food and fluid intake, risk assessments and complete HBPoS assessment tool.

To gather collateral information on clients from GP, community teams, liaise with other trusts (SPA/CATT/CMHT), family and friends if possible.

To contact S12 doctor and provide accurate and comprehensive handover of clients attendance and presentation within 1 hour of clients arrival.

To contact AMHP and advise of attendance, handover of presentation and complete necessary documentation to facilitate assessment.

To take a thorough and detailed handover from the psychiatric liaison department, ensuring all correct process on the ED diversion pathway have been followed.

Ensure all necessary polices and protocols are followed including the trusts S136 policy.

Ensure all necessary documentation is completed including risk assessments, 136 pathways, progress notes and S136 paperwork are recorded on the Trusts patient record RiO.

Ensure safe discharge/admission of client and that all follow up care of the client is arranged in a timely manner

Appropriately manage highly emotional, verbally aggressive, abusive or threatening clients and defuse such situations when appropriate, often under difficult and hostile circumstances.

Regularly assess clinical environment and report any issues highlighted
