Job summary

An exciting opportunity has arisen for a senior practitioner to join the Cognitive Impairment and Dementia Service (CIDS) in Hounslow.

CIDS is part of the Older Peoples Mental Health Service (OPMHS) line and has sister teams in three Boroughs, Ealing, Hammersmith and Fulham and Hounslow.

Hounslow is a small dynamic team that needs an enthusiastic professional to provide a leading role in offering patients with complex enduring mental health needs and their carers assessments treatment and care. The post holder will provide clinical supervision to team members, will monitor referrals, allocate work to colleagues, and liaise with GPs, social care colleagues, inpatient wards and others to ensure care is delivered to this vulnerable group.

The team base for Hounslow CIDS is located at West Middlesex Hospital Site.

Main duties of the job

Your role within the team will be to take a lead in implement treatment and care plans to address the health needs of our service users. You will also provide support to carers work within the multidisciplinary team and liaise with a variety of other teams and organisations. You will be in a leadership role and be involved in shaping the future developments in the Service

Candidates will need to demonstrate some understanding of the needs of older people with mental illness and physical health care challenges.

  • Registered Practitioner (mental health / LD/ Adult) appropriate to the job role
  • Degree/diploma level qualification supplemented by specialist qualification and/or experience.

Desirable

  • Courses to masters level equivalent relevant to the care group
  • Evidence of post registration training in relevant service area Completion of student Supervisor Course/equivalent/Practice Educators/

Experience

Essential

  • Extensive post qualifying experience in relevant service area
  • Experience of supervising staff and students
  • Experience of managing clinical risks with complex cases.

Desirable

  • Experience working with patients and carer groups to develop innovative practice.
  • Lived experience of mental health issues/caring for Older person with mental Health difficulties/dementia complex mental health needs/dementia
