Job summary

Do you want to work in a high performing team, are you organised and do you enjoy fostering good relationships? If so, we have an exciting opportunity for a Bed Manager to join the Bed Management Team who will be operationally based across our two inpatient units.

The post holder will be the central point for all referrals for inpatient admissions and will be working within an integrated team alongside discharge coordinator and bed management team to ensure effective patient flow which includes reducing the length of inpatient stay. They will also be responsible for ensuring comprehensive and coordinated discharge planning processes are in place including working with Delayed Transfers of Care. The post holder will also hold a key role in supporting with efficient transfer once a Decision to Admit has been made, as well as working alongside the Acute Trust to minimise any breaches.

Main duties of the job

The post holder will work closely with the Patient Flow Manager, and deputise where required, to manage the inpatient bed provision across the Adult Acute Inpatient Services in Hounslow and Hammersmith & Fulham including PICU and Older Adults Inpatient Services. The post holder will be expected to work closely alongside the Senior Management Team, Inpatient Leadership Teams and Community Teams to ensure effective patient flow in maintained.

Job responsibilities

The Bed Management Team is the single point of access for all referrals for acute inpatient admission. The post holder will co-ordinate and manage patient flow across Adult Acute Inpatient Services (including PICU and Older Adults Services) ensuring the best quality of care for each patient. The post holder will work alongside the Team Manager and Senior Management Team to ensure effective patient flow and a quality lead service.

  • Coordinating all bed requests and enable streamlined capacity management for all boroughs.
  • Provide support to ensure all admissions are purposeful face to face gatekeeping happens with a consistent threshold for admission overseen by the bed management team manager.
  • Ensuring compliance with the principles of the London Compact.
  • Clinical System recording and managing the Waiting List and DTA function.
  • Management and leadership of daily communications including leading bed management calls; ensure bed state is circulated by 5pm Monday to Friday, and out of hours to senior manager on call and senior nurse on call.
  • Oversight of all patients admitted across two sites including the recovery house (working alongside ECATT leads) as well as attendance at all bed management meetings.
  • Point of escalation for Surge Teams, disputes in relation to bed requests.
  • Central point for Out of Area Placements (OAP), ESR requests, coordinating relevant referrals and any required command calls.
  • Coordinate and support planning of, outcome of and change in circumstance for expected DTAs, bed requests, planned MHAA and police S136 calls.
  • Providing a consistent approach to all WL patients in regards to patient flow systems/processes and enable a seamless bed management/discharge coordination pathway.
  • Support with potential breaches to reduce escalation to out of hours on call manager and minimise bed escalations from A&E.

  • A relevant professional Qualification (e.g. RMN, Social Worker, OT) with an active registration.

Desirable

  • Degree in Health Related subject

  • Demonstrable experience of working in front line clinical service and understanding of pressures and ways of working.
