Job summary
We are looking for an experienced and motivated full time Specialty Doctor, to join our Specialist Community Forensic Team (Brent, Harrow and Hillingdon Hub), within the West London Forensic services, based at St Bernard's Hospital Ealing.
This is a full time fixed term post available from the beginning of August through to the beginning of April.
The salary range for this post is £50,373- £78,759 per annum plus London Zone Weighting.
Main duties of the job
You will be joining a well- established multi- disciplinary clinical team providing community care within our Specialist Community Forensic Team. The successful candidate will work with a multidisciplinary team under the supervision of a consultant forensic psychiatrist to provide effective, responsive, caring and safe treatment for our service users.
Each service user has a Responsible Clinician, Social Supervisor and CPN. You will work alongside this team to offer assessments and reviews in emergencies, assessments of service users referred to the service who are preparing for discharge from a secure ward and carry a small caseload of those preparing for transfer to the local CMHT.

The post holder will work closely with the responsible clinicians for the patients. You will be supervised by the consultant in the SCFT BHH hub. Other members of the multidisciplinary team include a clinical psychologist, CPNs, social workers, an occupational therapist, recover and peer support workers. There is also input from an arts therapy team providing art therapy, a family therapy team, a forensic psychotherapy team and the Drug and Alcohol Support Service.

The following is a guide to the posts responsibilities, and is subject to review:
Clinical role
- Contribute to assessment of service users referred to SCFT (both internally within WLFS and externally, e.g. from local psychiatric services). You may be able to lead on these at some point under supervision.
- Contribute to SCFT support of service users preparing for discharge with SCFT follow up.
- Provide medical input to the case management of community forensic patients within a multidisciplinary framework where changes to patient care are discussed in clinical team meetings. This will include routine reviews (with a multidisciplinary team member), CPA preparation, attendance and clinical note taking, crisis management and, when indicated, arrangements for the recall of conditionally discharged patients to hospital.
- Support the Responsible Clinician through preparing medication histories and preparing notes reviews.
- Gain specific experience in managing conditionally discharged service users with social supervisors and in liaising with the Ministry of Justice (including the preparation of quarterly reports in this context).
- Coordinate care with other agencies including referral of stable SCFT service users to local psychiatric teams, liaison with primary healthcare services and other agencies.
- Ensure timely and accurate documentation, including risk assessments and correspondence, including reports for MoJ and MHRTs.
- Undertake forensic risk assessments from local services.
- Attend at the weekly clinical team meetings, amber and referral meetings and monthly clinical governance and reflective practice meetings.
- Liaise with Pharmacy and Primary Care on issues affecting service user care, including arranging and following up annual or planned healthcare reviews with our West London primary healthcare team.
- Support the whole SCFT through leave cover for other specialty and ST doctors across the SCFT service
- Be encouraged to become involved in Clinical Audit, both individually and collectively and will contribute to the improvement of the quality of the clinical work of the service by adopting evidence based practice.
- Be committed to improving the quality of clinical care and will contribute to the Clinical Governance agenda within the service.
- Have experience in teaching and training to a multidisciplinary group of professionals.
The post holder will have 10 PA. The overall split of the programmed activities is 8 PA direct clinical care 2 PA of Supporting Professional Activities.
The post does not include participation on the on call rota but this opportunity may be available if the postholder wished to undertake these.
There is a regular forensic Academic Programme based on site and via MS teams. The local programme includes case presentations and reflective practice. In addition, both the Trust and Scheme organise periodic CPD events to which trainees are invited. Trainees are encouraged to review with their supervisor those opportunities to participate in internal or external courses and CPD events relevant to their training that may arise.
