We are looking for committed and enthusiastic nurses to join our Cognitive Impairment and Dementia Specialist (CIDS) which provides community based treatment and care to all adults with dementia. The role of Community Nurse is to work in partnership with patients and their families/carers in the Older People Community Mental Health Service, developing their personalised care plans to enable them to lead fulfilling lives.
Main duties of the job
To provide specialist assessments, reviews and nursing interventions to patients accessing CIDS. The post holder will demonstrate an understanding of clinical interventions relevant to the care group to include education, care plans and discharge planning, working closely with families/carers and partner agencies to facilitate this. The post holder will work at all times to promote the safety and the wellbeing of patients and their families/carers. They will provide clinical supervision to staff and students.
To ensure the successful implementation and roll out of care for dementia patients the post holder will demonstrate an understanding of clinical interventions relevant to Dementia care and will work at all times to promote the safety and the wellbeing of service users and their families/carers. The post holder will assess, plan and implement care, and provide specialist nursing advice and carry out specialist nursing procedures. They will provide clinical supervision to staff and students.
The post holder will be able to form therapeutic, meaningful relationships with patients and their families and carers, ensuring they are actively involved in both the planning and delivery of their care
