Job summary
NHS reforms and government directives have placed primary care in the forefront of health care provision. District nursing is undergoing a huge transformation with a multitude of new roles, responsibilities and career structures.
The senior professional development nurse will work alongside the lead district nurse for professional development, district nurse team managers and deputy team managers to enable nursing associates in the London Boroughs of Islington and Haringey to develop their clinical, interpersonal and communication skills, plus professional attitudes. The post holder will support the lead district nurse to ensure that regular service audits take place.
The post holder will assess and supervise trainee nursing associates in clinical practice and provide one to one supervision and teaching until the required level of competence and skills are achieved.
The post holder will be required to work across both Haringey and Islington and will be required to work occasional twilight and well as day shifts.
Main duties of the job
Managing Care
To have responsibility for the effective and efficient delivery of nursing care to patients as delegated by District Nurse Team Leader.
Work one to one clinically with the TNAs providing feedback to the individual on their performance as well as to their Practice Assessors/supervisors and team managers.
To undertake skilled nursing activities directly related to the assessment, planning and evaluation of patient care and the supervision and training trainee nurse associates.
To promote personal, professional, and clinical expertise through regular evaluation and updating of practice.
Work with the team managers to identify new practice assessors and supervisors and support them through the PA/PS programme.
Work with Academic providers, programme leaders and university link lecturers to ensure good communication and achievement of all formal assessment priorities' Act as a role model and peer support in the clinical environment.
To fulfil requirements of the Community Care Act, through effective liaison with Social Service Departments, Local Authorities and providers of acute care plus other agencies.
To undertake health promotion, including chronic disease management, for patients and their families on the current caseload.
To be involved in care/case management under the supervision of the District Nurse Team Leader, including of clients with continuing care needs.
