Job summary
The post holder will manage both the human and financial resources of a nursing team under the supervision of a named District Nurse Team Manager. She/he will be professionally accountable for the appropriate allocation of patient care to nurses within the District Nursing Team and for the supervision of their work. She/he will be responsible for the assessment, planning, implementation, evaluation of care of patients. She/he must at all times meet professional and clinical standards/ guidelines and be a leader and advocate for these within the team. The post holder will be work as a close link between the community and hospital to promote integration and continuity of care. The successful candidate will be required to work unsocial hours as required by the service (including weekend and evening working).
Main duties of the job
Managing CareTo have responsibility for the effective and efficient delivery of nursing care to patients as delegated by District Nurse Team Manager.To undertake skilled nursing activities directly related to the assessment, planning and evaluation of patient care and the supervision and training of junior staff and students.To promote personal, professional, and clinical expertise through regular evaluation and updating of practice.To ensure that practice is delivered in line with the NMC code at all times.To ensure that the 6Cs (including compassionate care) underpins own practice and is promoted as an ethos within the service.To assist in leading the team to meet the mission statement of the service and embed it into practice.To fulfil requirements of integrated working and care, through effective liaison with Social Service Departments, Local Authorities and providers of acute care plus other agencies.To undertake health promotion, including chronic disease management, for patients and their families on the current caseload.To identify any trained nurse within the team to be the named nurse for patients of the caseload.To be involved in care/case management under the supervision of the District Nurse Team Manager, including of clients with continuing care needs.
