Job summary

An opportunity has arisen to join our UCLH@Home Virtual Ward Team based at University College Hospital.

The successful applicant will demonstrate a high level quality care and evidence-based interventions as a member of the UCLH@Home Team.

The role will involve support for the nurse in charge and visiting nurses and to be able to perform a wide range of administrative and clerical duties autonomously and in close liaison with wider team. They will provide a wide range of skills and be able to take appropriate action to achieve the planned aims and objectives of individual clients.

This will involve being personally accountablefor the standards of such delegated duties. The post holder will continue to develop and undertake a varied range of delegated tasks and will report to a registered professional. The role will continue to develop through gaining further skills, knowledge, and competencies.

Main duties of the job

Main Duties:

  • Accurate and timely recording of data
  • Working closely with our UCLH@Home Team and other medical professionals to organise appropriate referrals and ongoing support to patients in their own homes
  • To communicate in a timely and comprehensive way with nursing colleagues and members of the Multidisciplinary team
  • To be competent in essential technical skills such a Microsoft Word, PowerPoint, Excel and Microsoft Teams
  • To provide administration support to the team
  • To organise own workload and establish priorities
  • To obtain patient satisfaction data and report on findings
  • To work independently and manage own workload using effective time management
  • To provide spontaneous and planned advice, teaching and instruction to service users, relatives and other organisations as required
  • To attend and contribute to team meetings and case discussions using effective communication
  • To work collaboratively with other agencies and organisations to ensure delivery of an equitable and integrated service
  • To initiate referrals to other agencies, including Social Services, where appropriate
  • To keep up to date and accurate patient records (including electronic records where required)
  • To participate in regular team meetings / meetings. This may include minute taking as required
