Job summary

This is an exciting opportunity for a senior clinical fellow in acute medicine who are considering a career in acute medicine or gaining further experience in acute and general internal medicine. This post supports the existing Acute Medicine tea in delivering high quality care to patients at the front door of the hospital and will enable you to further develop clinical knowledge and skills across the general internal medicine curriculum.

You will work with a dynamic multidisciplinary team. Your training will mirror that given to doctors in training programs within the department and you will be allocated a consultant supervisor to support your career development. You will be encouraged to grow as a clinician and participate in governance including mortality reviews, QIPs, departmental and trust-wide educational activities.

Main duties of the job

It is suitable for doctors who are considering a career within the specialty or in a related medical specialty, or who simply wish to develop their skills & knowledge in adult medicine at a senior level. You will be part of the Acute Medicine team who deliver high quality care to patients at the front door of the hospital & will have the opportunity to practice procedural skills, develop your clinical acumen & leadership, reflecting the HST generic & GIM curriculum.

You will be allocated a consultant supervisor to support your career development. You will be encouraged to grow as a clinician & participate in governance including mortality reviews, QIPs, departmental & trust-wide educational activities.

You will work with a dynamic multidisciplinary team & play a role in teaching medical students & junior doctors, work collaboratively with the START team (accelerated discharge therapy team), Virtual ward, frailty team as well as the other hospital based specialties & tertiary services. Effective communication skills form a key part of this role, & this is an excellent opportunity to step up to the next level of clinical leadership & decision making from an SHO level role.

There is a large team of consultants with a spread of interests within & outside Acute Medicine, including Geriatrics, ITU, Oncology, Clinical Pharmacology, Education, & Maternal Medicine. You will have the opportunity to link in with these other interests if you wish as part of your training.

Job responsibilities

The postholder will work across the following 3 areas as outlined in the work schedule below:

  • the Acute Assessment Unit (AAU), Mary Seacole North and South wards
  • the Acute Medical Take
  • the Ambulatory Emergency Care Unit (AEC) to facilitate same day emergency care (SDEC)

Acute Assessment Unit (AAU)

This is a 34-bed unit based across two adjacent wards (Mary Seacole North and Mary Seacole South). This is staffed on a daily basis by 2 Consultants, a registrar, 2-4 SHOs and 2-3 FY1s with daily Consultant led ward rounds.

Wider AMU staffing includes a dedicated and highly trained nursing, physiotherapy, occupational therapy and MDT team, a frailty team, pharmacy and a full complement of support staff.

There are full cardiac monitoring facilities and isolation facilities, and patients may receive non-invasive ventilatory support on the AAU as well as chest drains, management of level 0 and level 1 patients and a close working relationship with the intensive care, cardiology, respiratory and gastroenterology teams who all in reach to AAU regularly.

Ambulatory Emergency Care (AEC)

The award-winning Ambulatory Emergency Care Unit is staffed by a dedicated team of senior and junior doctors and Advanced Clinical Practitioners who see acutely unwell patients, in an outpatient same day emergency care (SDEC) setting, providing an alternative to admission for many patients.

The ambulatory care unit has a dedicated Consultant and Senior Nurse who supervise the diagnosis, management and treatment of patients on the unit and provide teaching and support for the many procedures (chest drains, ascitic drains, lumbar punctures, infusions and diagnostic imaging) which take place there.

Acute Medical Take

This is staffed by 1 SpR, 2 SHOs and 1 FY1. There is an average intake of 30 patients over 24 hours. During the day, there is a continuous consultant post take ward round providing the opportunity for immediate feedback and learning. Overnight, Consultant advice is available by telephone, and patients are reviewed on the morning post take ward round.
