Whittington Health Community Respiratory Team is looking to recruit a Band 3 Community Health Care Support Workerto work alongside our Respiratory Physiotherapists and Specialist Nurses.

Main duties of the job

You will be fully supported in assisting in the delivery of Pulmonary Rehabilitation classes, Spirometry and mainly supporting patients with COPD. (Working in clinics across Islington and You will also have the opportunity to work with the Hart Failure and Diabetes specialist teams

You will be able to attend our in-house assistants training course to help you to develop your skills and experience in order to confidently assist respiratory patients and help them better manage their condition.

Job responsibilities

MAIN DUTIES

  • To work within a multi-professional team and provide support to the team of nurses and physiotherapists
  • To report any changes in a patients condition to a member of staff
  • To develop skills needed to provide high standard spirometry and deliver spirometry clinics for patients across Haringey
  • To co-ordinate the booking and discharge of pulmonary rehabilitation patients
  • To co-ordinate the booking, assessment and discharge of spirometry patients
  • To assist in the delivery of pulmonary rehabilitation for patients in a group setting alongside the physiotherapist. Taking an active role in the assessment, education and exercise components of the group sessions
  • To support specialist nurses/AHPs on home support/case management which may involve domiciliary visits to patients homes.
  • To implement treatment and care to patients under the guidance and supervision of a registered nurse/physiotherapist
  • To communicate accurately and effectively with GPs and referrers regarding patient referrals following clinic appointments
  • To be responsible for administrative support within the team as required.
  • To practice the principles and philosophy of individualised person centred patient care in accordance with Trust Policy, procedures and regulatory bodys guidelines
  • To record activity on a clinical database as required and maintain accurate and contemporaneous clinical records of given care.
  • Participate in ordering and monitoring of stores, equipment and other appropriate tasks.
  • Develop and maintain skills required for patients within a community setting in line with appraisal and service objectives.
  • To participate in supervision and appraisal and take responsibility for own practice development.
  • Occasional periods of cover may be required across sites for pulmonary rehabilitation.
