Job summary

This is also an ideal secondment opportunity for those with acute cardiac experience to develop there skills and/or those with community heart failure experience.
This involves identification, management, and interventions for adults at risk of developing/with LTCs in Haringey's neighbourhoods. Main duties of the job
The heart failure nurse will help provide a high-quality specialist nursing service for heart failure (HF), with the primary focus in delivering the health inequality work in Haringey but working alongside the existing heart failure team to residents in Islington and Haringey.
You will manage a patient caseload and promote excellence in clinical nursing skills through the provision of expertise in HF to peers and community carers, working closely with other members of the community HF team, the relevant cardiologists and Whittington Health / North Middlesex University Hospital Consultant HF leads, and general practitioners.
You will also facilitate the implementation of agreed HF care to patients, through close collaboration with the CCGs, the extended HF team, general practice, and Whittington Health HF Lead.
This post is available due to the expansion of the well-established heart failure nurse team.
