Job summary
An exciting opportunity has arisen for a Band 4 Community Health Care Support Worker to join our Heart Failure inequalities project team and Heart failure Specialist Service at Whittington Health. The project involves supporting patients who may face challenges engaging with healthcare professionals specialising in Heart failure. Within this role the post holder will support registered healthcare professionals to deliver Heart failure care. There will be elements of patient contact, ensuring that the administration involved in care delivery is completed and supporting in facilitating some group consultations as delegated by the Specialist teams
Main duties of the job
- To work within a multi-professional team and provide support to a team of health care professionals
- To support specialist nurses in clinic settings and on home support/case management which may involve domiciliary visits to patient's homes.
- To be responsible for administrative support within the team as required
