Job summary

The post holder will be a key member of the Surgery and Cancer Integrated Clinical Service Unit who will offer support, problem solve and deliver on the implementation of cancer personalised care across Whittington Health NHS Trust in collaboration with primary care, North Central London Cancer Alliance, Macmillan and volunteer sector. In partnership with key multidisciplinary teams the post holder will operate within a project management methodology to plan, develop, and support the delivery of personalised interventions from diagnosis to end of treatment. As a result, these interventions will improve patient care and patient experience through enhanced quality of services delivered at best value. They will work in collaboration with the clinical teams, cancer information and support services, allied health professionals and key stakeholders to embed personalised care into patient pathways.

Main duties of the job

The poster holder will:

  • Utilise project and change management techniques & methodology to support long term objectives of the project
  • Utilise advanced knowledge and skills & experience relating to the speciality to provide specialist advice & expertise to other members of the multidisciplinary team
  • Be able to produce detailed documentation to support specific business plans for presentation to colleagues & senior management
  • Demonstrate excellent communication skills, which facilitate working in partnership with other teams & with external organisations.
  • Use negotiation and persuasion skills to ensure colleagues & stakeholders understand the importance of projects and work towards timescales
  • Coordinate, lead & be accountable for the implementation of this programme of work
  • Support the Cancer Nursing Teams in ensuring that Personalised Care & Support plans based on electronic Holistic Needs Assessments are offered to all cancer patients
  • Work with the clinical teams to ensure End of Treatment Summaries are completed
  • Develop the provision of Health & Wellbeing Information & Support,
  • Be the catalyst for supporting teams to design and implement Personalised Stratified Follow-up pathways
  • Work collaboratively with patients, NCLCA, Macmillan and volunteer sector
  • Monitor & report related performance targets
