Locality Therapies Community Physiotherapist - Band 6 (37.5hrs/week)
Base: Lordship Lane Primary Care CentreOuter London weighting: Full-time.Mon-Fri 9-5pm. We are looking for a therapist with junior core rotational experience to work in the community.

We are looking for an experienced, enthusiastic clinician to join our Haringey team of physiotherapists, occupational therapists, speech therapists and rehabilitation technicians. The team works with adults of all ages and is based on achieving client-centred goals.
Main duties of the job
To provide high quality effective physiotherapy assessment and falls treatment/management programmes for adults presenting with a range of conditions in Haringey, who require a community based service.To use specialist physiotherapy knowledge and skills to manage own complex caseload, as an autonomous practitioner.To act as a source of expertise on falls, orthopaedic and neurological conditions and rehabilitation liaising closely with specialist services; providing advice to patients, carers, all grades of physiotherapists, other health care professionals, consultants, general practitioners and social services.To support the use of evidence based practice within the service, incorporating national guidelines e.g. falls guidelines into practice.To assist with the running of strength and balance classes in the community.

CLINICAL
- To deliver patient care based on principles of best practice, achievable through direct patient contact or via supervision.
- To carry out specialist, comprehensive assessments of patients with a complex presentation, using investigative and analytical skills formulating individualised management and treatment plans.
- To provide specialised physiotherapy interventions at home or in a community setting as required, collaborating with colleagues in the community and secondary care health professionals to promote continuity of care.
- To ensure a timely response to hospital discharges in line with Haringeys Trust policies.
- To provide specialist assessment for and provision of multi-disciplinary equipment in line with national and local guidelines.
- To undertake joint visits with all multi-disciplinary team members where appropriate and provide specialist advice on treatment/ management regimes.
- To keep accurate electronic treatment records, which fulfil legal and statistical requirements and which are in accordance with HPC, CSP, Trust and ICTT documentation standards.
- To facilitate and lead weekly falls exercise classes working closely with multi-disciplinary team, and collating data for ongoing audit/monitoring as required.
- To attend and actively participate in MDT clinical meetings.
- To challenge and improve current practice where appropriate, ensuring practice is evidence based.
- To be able to gain consent, often in the presence of communication difficulties, and be able to work within a legal framework with patients who lack capacity to consent to treatment.
- To be involved in public health promotion. Sometimes producing materials to facilitate effective delivery of treatment and ensure compliance and carry over.
EDUCATION
- To identify own learning needs related to clinical knowledge and skills in the management of patients following stroke and with long term neurological conditions.
- To support in the delivery of multidisciplinary education and training.
- To access further education programmes to develop own knowledge and skills, by keeping up-to-date with current best-practise in physiotherapy (particularly in areas of community rehabilitation, falls and neurology) and to share learning with the wider team.
- To provide specialised advice, teaching and training to other members of the MDT regarding the physiotherapy management of adults in the community.
- To actively participate in the delivery of student education as a Clinical Educator.
MANAGEMENT
- To co-ordinate workload of self and assistants as required.
- To participate in service reviews and policy, guideline and protocol development as required by NHS Haringey and other services.
- To support the provision of evidence based clinical practice within physiotherapy and the community rehabilitation service.
- To ensure that all equipment and treatment resources are optimally used for the management of patients with neurological conditions and across the community service.
- To ensure that all relevant policies are adhered to and assist the clinical leads in any investigation or actions to improve performance.
- To participate in the supervision, education and training of junior physiotherapist, rehabilitation assistants and other members of the multidisciplinary team.
RESEARCH/AUDIT
- To support in the monitoring of the effectiveness of the community therapy service through the collation of data for ongoing audit.
- Contribute to the evaluation of research findings and disseminate them appropriately, including presentations at study days or conferences.
- To support the monitoring of adherence to local audits on neurological conditions.
PROFESSIONAL RESPONSIBILITIES
