We are looking for an enthusiastic ACP to join our team and support patients to stay at home, reducing the need for hospital admission.

The ACP will work as a member of RAS which includes a rapid response team supporting admission avoidance , a frailty virtual ward and a therapy urgent response team supporting hospital discharges. The virtual ward enables patients to receive safe, effective treatment at home for up to 14 days. They will provide medical assessments at home alongside our GPs, with overall care under a hospital consultant or community geriatrician . We receive referrals from community services, LAS 111, GPs and ED deflections with a focus on keeping patients safe at home where possible. A confident communicator with strong organisational skills, you will thrive in an MDT environment and will be instrumental in supporting and developing this new service, to ensure a high-quality service for our patients. With a passion for continuous development and service improvement, you will be involved in projects and trust initiatives. You will receive regular supervision, in-service training, and support.
Main duties of the job
The post holder will proactively manage a caseload of patients requiring complex case management in order to prevent hospital admissions and facilitate early discharge to improve patient health outcomesThe post holder will be expected to develop skills in physical assessment, diagnostic reasoning and prescribing to assist in the recognition of early symptoms of disease exacerbation and acute illness. The post holder will be expected to co-ordinate care and navigate the patient through pathways across the primary, secondary care interface, including the prevention of unnecessary admissionsThe post is responsible for the development and implementation of clinical therapy and intermediate care pathways across the urgent community response teams across Islington and Haringey. They will ensure that governance arrangements are in place which are clearly communicated to the clinical teams.To fulfil the role of ACP and Clinical pathway Lead, acting as an expert advisor for the team, in the assessment and rehabilitation of adults in the community with highly complex physical disabilities.To ensure that all therapists in IUURT and HURRT maintain high clinical and professional standards of practice.To ensure that all staff achieve and maintain appropriate levels of education and continuing professional development.To keep up to date with developments in urgent response and intermediate care, undertaking audit to evaluate practice and participating in research as appropriate.

- Degree qualification in physiotherapy /OT/SLT
- MSc qualification in Advanced clinical practice or willingness to work towards it. E E E

- Evidence of experience supervising staff
