Job summary
This post of Senior Clinical Fellow is a joint post, providing training in both Diabetes & Endocrinology and General Internal Medicine (GIM) at the Whittington Hospital NHS Trust
The post-holder will participate in the acute medical take as Duty Medical Registrar (DMR). Acute admissions are admitted to the Acute Assessment wards (Mary Seacole North and Mary Seacole South) and are transferred after the post-take ward round to the ward most appropriate to their needs. There are acute Consultant physicians present 8 am to 8pm, to support the acute take Monday to Friday. Weekends are similarly supported by consultants in medicine across the Medicine ICSU.
The post-holder will join the team providing inpatient care on the ward led by the Diabetes & Endocrinology Consultants, currently Coyle Ward. Most patients transferred to this ward are elderly with multiple problems and multiple co-morbidities. There are close relationships with the multidisciplinary supporting team and with other specialists in the Whittington.
Main duties of the job
The post-holder will undertake 1 - 2 Diabetes and 1 - 2 Endocrinology outpatient clinics per week under the supervision of the consultants. Clinics will be cancelled at the time of other responsibilities, including on-call duties and inpatient ward work. Diabetes patients seen will include patients with type 1 diabetes and patients with complex type 2 diabetes. A full supporting Diabetes MDT is available for outpatient work. Endocrine patients seen will include patients with a broad range of endocrinological conditions. Consultant supervision will include a post-endocrine clinic team case discussion, which the post-holder will attend.
The post-holder will also provide specialist Diabetes & Endocrine review and management advice for Whittington inpatients under various teams. For Diabetes inpatient care, the post-holder will work with the Lead Inpatient Diabetes Specialist Nurse. For patients requiring tertiary level management, the post-holder will liaise with in-reach Consultants from the Royal Free NHS Foundation Trust, including Vascular Surgery and Renal Medicine.

The post-holder will take part in the acute general medical on-call rota, acting as Duty Medical registrar (DMR). There are 2 SpRs on call per 24 hours Monday to Friday with an additional ward cover SpR at weekends and Bank Holidays.
The post-holder will support the junior medical team of the Endocrinology Ward Consultants (currently Dr Maria Barnard, Dr Deepa Chandran, Dr. Artemis Vogazianou, Dr. Julian Waung) in the care of patients admitted to their ward, currently Coyle Ward. The junior doctor team on Coyle consists of FY1 doctor x 2, GPVTS doctor and the SpR. The post-holder will share the SpR duties with the Renal SpR and the other Diabetes & Endocrinology SpR(s).
The post-holder will attend the Diabetes & Endocrinology Clinical Radiology Meetings, where inpatient imaging investigations are discussed.
The post-holder will attend the Diabetes & Endocrinology Academic Departmental Meetings and the weekly Department of Medicine Grand Round.
The post holder should be prepared to perform duties in occasional emergencies and unforeseen circumstances. Commitments arising in such circumstances are, however, exceptional and the post holder will not be required to undertake work of this kind for prolonged periods or on a regular basis. All efforts will be made to ensure that work of this kind does not result in continuous hours of duty which exceed the New Deal continuous hours of duty limits. Junior doctors are contractually bound to work with the Trust to achieve and maintain New Deal compliance.
