Job summary

A Band 5 to 6 Development Specialist Speech and Language Therapist. We will consider applications from speech and language therapists interested in developing their skills to take on the role of Specialist Speech and Language Therapists e.g. staff currently working within acute services without community experience.

The post is based in the Islington Community Rehabilitation Team, which is a community multidisciplinary rehabilitation team in Islington.

The post holder will be part of a team responsible for delivering a comprehensive and specialist therapy service to patients, families and carers of adults with neurological, respiratory or surgical conditions, and elderly based in Islington Nursing Homes, referred with communication and swallowing difficulties. S/he will need specialist knowledge in the assessment, differential diagnosis and treatment of swallowing and communication difficulties.

The post holder is responsible for organising and managing a defined workload, within their professional boundaries, seeking advice where indicated.

Main duties of the job

S/he will assess clients and design and supervise therapy programmes to be carried out by clients, rehabilitation health care assistants, nurses, student, volunteers and other carers. S/he will provide clinical support to less experienced colleagues within the team. S/he will monitor the work of, and work with, assistants . S/he will contribute to policy development and the development of clinical protocols.

The post holder will be responsible for developing, delivering and evaluating training as required.

S/he will participate in the clinical education of students who are on placement with the team as required.

The post holder will contribute to any research and/or audits being undertaken as part of this project or within the team / service as required.

Since April 2011, Whittington Health operates as an Integrated Care Organisation (ICO) which offers healthcare across acute and community in Islington and Haringey.

The post holder will be part of a team responsible for delivering a comprehensive and specialist therapy service to patients, families and carers of adults with neurological, respiratory or surgical conditions, and elderly based in Islington Nursing Homes, referred with communication and swallowing difficulties. S/he will need specialist knowledge in the assessment, differential diagnosis and treatment of swallowing and communication difficulties.

The post holder is an autonomous practitioner and is responsible for organising and managing a defined workload, within their professional boundaries, seeking advice where indicated.

Main Duties

The main Duties and Responsibilities of this post are:

  1. To provide a community Speech and Language Therapy service to adults living in Islington, including Care Homes. Travel will be required, which may be made on foot, by public transport, bicycle, motorcycle or car. To ensure that all reasonable precautions are taken in evaluating and reducing risk of lone working.
  2. To fulfil the role of a specialist Speech and Language Therapist, offering specialist input, in the assessment, rehabilitation and management of adults in Nursing Homes with complex swallowing and communication difficulties, including knowledge of AAC and LSVT.
  3. To ensure that specialist clinical and professional standards of practice are maintained, including ensuring the maintenance of appropriate levels of education and continuing professional development in acquired swallowing and communication disorders, seeking advice where indicated.
  4. To offer regular clinical support to junior staff working within the team. The post holder will undertake regular clinical supervision with the Highly Specialist SLT in the Islington.
  5. To promote the interests of the SLT service and their clients both within the local team and beyond. To ensure close working with patients, families, carers and members of the MDT to ensure collaborative working, and to set, monitor and review goals for intervention.
  6. To contribute to the continued development of clinical and strategic pathways across the ICO to enhance patient journeys and optimise care, working with colleagues in acute and other community and social agencies.
  7. To contribute to specific projects in line with the needs of the SLT service and within the community.
  8. The post holder will have an understanding of clinical governance, and keep up-to-date with developments in adult community rehabilitation, with special involvement with communication and swallowing difficulties, contributing to audits to evaluate practice, and contributing in research projects.
  9. The post holder will develop an awareness of current policies and practice in health care and social services e.g. Adult Safeguarding
  10. To contribute to clinical effectiveness by use of evidence based practice and outcome measures
  11. To ensure working practices are supported so that all the disciplines working in the Teams can develop flexible interdisciplinary/multi-agency work practices, to reflect and support productivity initiatives and different working practices when required, which enhance outcomes for clients .
  12. S/he will also contribute to the development and delivery of lectures to undergraduate and postgraduate students. S/he will participate in the clinical education of students who are on placement with the team, both SLT and other AHP students.
