Job summary

Are you a motivated, reliable individual with experience in community settings? We are recruiting a Care Navigator to join our supportive team within Integrated and Community Care Services at University Hospital Coventry and Warwickshire.

Care Navigators play a vital role in helping people live independent lives though connection with their community. You will put people at the centre of every interaction; exploring what matters most to them through holistic wellbeing assessments and facilitating access to health, social care, and community resources. You will empower individuals to achieve their goals.

Within an Integrated Neighbourhood Team, you will contribute towards high-quality, alongside nurses, therapists, and social care colleagues. To support your development, you will receive monthly supervision from the Care Navigation Lead and an annual values-based appraisal.

We're looking for someone with excellent communication skills, the ability to work independently and a passion for helping people live safer, more independent lives.

The team currently operates Monday to Friday, 8:00-16:00, with flexibility for future service needs. The role is currently based at Newfield House, Kingfield Road, Coventry, with some travel to other sites.

Main duties of the job

Care Navigators work flexibly across the community to ensure individuals receive timely, coordinated support. The role involves gathering detailed information to understand a person's circumstances, identifying barriers to wellbeing, and working with them to develop clear, achievable plans. You will communicate regularly with a wide range of professionals, ensuring updates are shared and actions progressed to keep people safe and supported.

You will be responsible for organising and prioritising your own caseload, responding to changes in need, and ensuring appropriate escalation when concerns arise. The role includes maintaining accurate records, monitoring progress against agreed goals, and adapting support as situations evolve. Care Navigators also help individuals access practical solutions--such as equipment, community groups, or voluntary sector services--while encouraging confidence, self-management, and independence.

You will contribute to improving patient flow by helping prevent unnecessary admissions and supporting timely discharge back into the community. The post requires confident decision-making, effective communication, and a proactive approach to problem-solving to ensure every person receives personalised, meaningful support.

Successful applicants will be required to provide an immunisation and vaccination report, from either their current UK occupational health provider or their UK GP.

  • To work collaboratively with health, social care and the private sector signposting and facilitating referrals, enabling the provision of care from a variety of agencies. the post holder will provide central liaison between the services and carers, monitoring the care and movement of patients through services on recommendation following a clinical assessment and clinical triage.
  • Support clinical triage to meet needs of patient and delivery of service outcomes.
  • Deliver face to face and telephone contacts with patients to support interventions inclusive of; home visits to gather a comprehensive history and, in conjunction with hcps, determine their health & social prescribing needs; completing baseline observations and reporting back to clinicians; reviewing packages of care, goal plan progress, skin bundles and checks; caseload management and co-ordination of patient care.
  • Identify support available within required services across pathway, liaising with staff as required to support patient flow and escalating when appropriate. liaise with specialities to identify support and allocation of urgent visits.
  • To support clinical staff with tasks that aid in patient flow through the health and social care system.
  • Viewing, accessing and processing data from multiple trust information systems, and where possible 3rd sector & GP systems, to support care navigation.
  • To establish and maintain effective referral and communication between internal and external services to support care navigation
