Job summary

The Lead Pharmacist - Community Services and OCIT at UHCW NHS Trust plays a pivotal role in supporting integrated, community-based and out-of-hospital models of care. This post provides strategic and clinical leadership for pharmacy services supporting OCIT, Virtual Wards and other community-facing pathways that promote admission avoidance, early supported discharge and care closer to home.

Working closely with multidisciplinary teams, system partners, and community services across Coventry and Warwickshire, the postholder will ensure safe, effective and patient-centred medicines optimisation across community and transitional care pathways. The role supports service transformation, medicines governance and the safe transfer of care between acute, community and primary care settings, while contributing to strategic development aligned with Trust priorities and integrated care system objectives.

This role requires strong clinical leadership, advanced knowledge of medicines use across acute and community care settings, and the ability to work collaboratively across organisational boundaries to improve patient outcomes and deliver innovative pharmacy services.

Successful applicants will be required to provide an immunisation and vaccination report, from either their current UK occupational health provider or their UK GP.

Main duties of the job

The post-holder is responsible for providing clinical and strategic leadership for pharmacy services supporting OCIT, Virtual Wards and community-facing care pathways. To deliver safe, effective and high-quality pharmaceutical care to patients managed through admission avoidance, early supported discharge and care closer to home models.

The postholder will lead the development and delivery of medicines optimisation strategies that support safe care across community and home-based pathways, with a focus on reducing medicines-related harm at transitions of care and ensuring safe and effective medicines use outside of the traditional hospital setting. They will also oversee pharmacy input into evolving out-of-hospital pathways, ensuring robust medicines governance, compliance with legislation, and alignment with local and national guidance.

Working collaboratively with multidisciplinary teams and system partners across acute, community and primary care, the role will support integrated medicines-related care and strengthen medicines transfer of care processes. The postholder will also contribute to service development, clinical guideline implementation, quality improvement activity, and the education and development of pharmacy staff and the wider healthcare workforce.
