Job summary
You will be support the Renal Social Worker to work with patients with chronic kidney disease, providing practical support to kidney patients to ensure maximum engagement with treatment plans, clinic appointments and other appropriate interventions. You will be supporting young adults with chronic kidney disease to help with their transition and engagement into adult chronic kidney disease services. You will be responsible for assisting with the delivery of patient centred care under the supervision of a social worker. Provision of welfare and social support and assistance in benefits, claims and appeals is central to the role.
You will be expected to work collaboratively with key partner organisations, service users, carers, clinicians and other practitioners within the multi-disciplinary team in delivering the service
A full UK driving license is essential for this post. Please note that an international driving license is not accepted.

Successful applicants will be required to provide an immunisation and vaccination report, from either their current UK occupational health provider or their UK GP.
Main duties of the job
- To work with young adults in engaging with their treatment plan, clinic appointments and other appropriate interventions to optimise and maximise their personal safety and well-being, working holistically and with other professionals where appropriate.
- To participate in young adult and transition clinics and ensure timely support given to young adults and their family and carers.
- To support renal patients with social and welfare needs including education, employment, housing, finances and maintaining relationships.
- 4. Advise, assist and support renal patients to make timely welfare benefit claims, appeal and challenge welfare benefit decisions to help improve their quality of life.
- Advising renal patients and their families about available resources and make referrals to appropriate agencies for support.
- To assist renal social worker with identified areas of support and provide timely and appropriate feedback.
