Oncology Cancer Navigator (Urology)

Job summary

We have an exciting opportunity for an enthusiastic, proactive person to join our Oncology Team as a Band 4 Cancer Navigator specifically to support our Urology Cancer patients through their Oncology treatment pathways.

The post holder will be required to work autonomously in managing patient initiated follow-up which includes maintaining the remote monitoring database. You will also need to be able to demonstrate excellent communication skills either over the telephone or face-to-face, where you will be responsible for carrying out Holistic Needs Assessments, sign-posting patients appropriately, escalation of complex issues to the Clinical Nurse Specialist Team as required and liaising with service users and multi-disciplinary teams,

The ideal candidate will be confident, self-motivated, be able to manage their time effectively, with the ability to display advanced IT, administrative and organisational skills. Competence in data collation and input is also required. Commitment and compassion for patients and the quality of care they receive will be imperative.

Main duties of the job

The Oncology Cancer Navigator (Urology) will form an integral part of the multidisciplinary team in caring for the physical, social, and psychological needs of patients and carers diagnosed with urological cancer, throughout their treatment pathway.

The post holder will have daily direct contact with patients and their families, providing support, advice, signposting, and assistance with the Living with and Beyond Cancer agenda. This will include the arranging and triaging of holistic needs assessments as part of their core role.

The post holder will be required to work autonomously in managing patient-initiated follow-up which includes maintaining the remote monitoring database, ensuring that all investigations are booked and performed as per guidance, results are escalated as required and MDT meetings are arranged and held as appropriate.

The post holder will work as part of the Urology multidisciplinary team under the guidance and supervision of a registered practitioner.

  • To work with the CNS teams to develop and embed the Living with and Beyond Cancer agenda within the Uro-Oncology services.
  • Arranging and triaging of the electronic Holistic Needs Assessments (eHNA) during the cancer patient pathway. This will include the navigator assisting the patient with completing their eHNA where appropriate. This will ensure that patients are signposted in a timely manner to appropriate services.
  • The role will require highlighting any potential delays in the patient pathway and working to mitigate the impact of these problems to the patients. The post holder will be required to work with the senior staff to support and ensure that the patient and carer experience is central to every stage of their journey.
  • To ensure that patients who are are part of patient initiated follow-up are identified and the agreed pathways are implemented, this will included maintaining the remote monitoring system, ensuring that it is up to date and accurate.
  • Ensure the appropriate management of patient information and the co-ordination of patient pathways through collaborative working with medical secretaries, MDT coordinators, clinical nurse specialists and other departments as required within the Trust.
  • To assist in the development of end of treatment summaries alongside the CNS to ensure the timely communication of patient information to the primary care setting.
  • To support the Urology CNS Teams by responding to the non complex workload and referring to the CNSs as appropriate. Liaising and referring to other appropriate healthcare professionals.
