Main duties of the job
The post holder will work as part of the Community Palliative Care Team and undertake a broad range of nursing support duties relating to the care of patients with end of life care needs in the place where they live, usually their own homes.
The post holder will work under the direction of the Palliative Care Team providing care as required to patients with palliative and end of life needs. The post holder will work within the clearly defined boundaries of their competencies.
Please be advised that a Full valid UK driving license and access to a vehicle is essential for this post.
