This is an exciting opportunity for the right candidate to join our expanding advanced practice workforce within Emergency Medicine. The successful candidate will work under clinical supervision to meet specific speciality capabilities and competencies alongside successful completion of MSc in Advanced Practice, progressing to a Band 8a.

The post holder is an experienced healthcare professional able to undertake a Masters programme in Advanced Clinical Practice, who will work as a trainee Advanced Clinical Practitioner (tACP) within the acute medicine footprint. The tACP will assess non-elective patients including initial history taking, clinical assessment, diagnosis, treatment and evaluation of care with clinical supervision from senior clinicians; a Senior Advanced Clinical Practitioner/ Consultant Nurses/ educational supervisor. The tACP should be directly or indirectly supervised at all times until their competence has been assessed by the group ACP faculty. They will demonstrate safe clinical decision-making and expert care, for patients providing appropriate clinical management plans and prescription of any medication required.

Main duties of the job

The post holder will demonstrate critical thinking in the clinical decision-making process. They will work collaboratively with the Acute Medicine team to meet the needs of patients,and will play a key role in service delivery and adherence to policy and procedures. They will also be instrumental in guiding junior medical and nursing staff in relation to relevant protocols and procedures.

The post holder will be required to work within all areas of Acute Medicine. This includes Medical Assessment Unit (MAU) Primary Assessment and Treatment (PAT), Same Day Emergency Care (SDEC) Short Stay Wards undertaking primary and secondary assessments alongside medical colleagues, providing support and guidance to nursing and medical staff in the management of the patients within acute medicine. They will work alongside medical, nursing and multi-disciplinary colleagues in the management of these patients providing support, guidance and advice as required.

The post holder will be required to ensure evidence based care is delivered to patients and will be involved in audit of practice, research and service improvement.

Following completion of the MSC in advanced practice including the non-medical prescribing module, Individual competence and capability will be met in order to achieve a satisfactory level of competency evidenced by a successful ACAT assessment. Upon completion of these requirements the trainee will progress to band 8a.

Clinical

Co-ordinate the care of the acute medical patients throughout the care pathway with the multidisciplinary team and clinical services to ensure the effective management of this group of patients.

Assist the clinical and nursing teams, at all levels in the assessment and prioritisation of patients requiring intervention.

Use advanced clinical assessment skills under clinical supervision to manage patients with complex needs presenting to the Emergency Department with an undifferentiated diagnosis to ensure that optimum physical and psychological care needs are met.

Proactively develop treatment plans for patients requiring urgent intervention including fluid resuscitation, oxygen therapy, antibiotics or other urgent treatment.

Undertake procedures such as cannulation, venepuncture, arterial blood gases to facilitate efficient urgent care in the context of acute/chronic illness or sudden deterioration

Initiate relevant investigations such as X-ray, Ultrasound, ECG and blood tests to enable timely diagnosis and facilitate early decision making.

Prescribe and review medication, appropriate to patient needs in the context of acute illness, taking into account any treatment given previously for pre-existing medical conditions, providing information and advice to patients on prescribed medication regimens, side-effects and interactions.

Promote and deliver evidence-based care for patients presenting to Acute medicine.

Communication
  • Document clearly all aspects of assessment, diagnosis and management plan clearly and present the findings to the Consultant in charge in acute medicine.
  • Use the SBAR approach to communicate clinical concerns to the senior medical staff in the event that a patient is found to be critically unwell or deteriorating clinically
  • Communicate complex information effectively to patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating
  • Maintain effective communication with the multidisciplinary team both within acute medicine, emergency department and other departments
  • Act as an advocate for patients and colleagues
  • Ensure awareness of sources of support and guidance (e.g. PALS) and provide information in an acceptable format to all patients, recognising any difficulties and referring where appropriate
Delivering a quality service
  • Ensure clinical practise is research evidence based
  • Prioritise, organise and manage own workload in a manner that maintains and promotes quality
  • Critically evaluate and review innovations and developments that are relevant to the area of work
