Job summary

To provide comprehensive and confidential administrative and clerical activities to meet the needs of the Department in accordance with Departmental, Trust and National standards, policies and procedures. To effectively communicate related information and advice to enable the multi disciplinary team to achieve operational excellence.

Main duties of the job

MDT and Meetings

  • Coordinate and administrate the weekly Palliative Care MDT.
  • Team Meetings, including regular nurse meetings
  • Coordinate and administrate all relevant Governance meetings including 'Learning from Deaths' and QIPS

Patient Care Coordination

  • Team coordination to support triage of new referrals and updating EPR and relevant database(s)
  • Ensure patient dashboard is up to date to support daily caseload meeting
  • Manage daily bereavement process
  • Triage calls from patients, families and referrers ensuring messages are appropriately and sensitively communicated to the team

Job responsibilities

Quality and Audit

  • Coordinate and administrate palliative care audit programme including support for NACEL National Audit of Care of the Dying Patient, including the procurement of patient notes (where applicable)

Education

  • Coordinate Palliative and End of Life Education programmes including advertising, booking, sending course information and processing evaluations
  • Coordinate Lunch and Learn Programme
  • Liaise with Medical School to coordinate medical student placements

Team coordination

  • Coordinate new staff inductions
  • Field and triage all telephone enquiries
  • Update team diary
  • Update team list/bleep numbers as well as contact details for other palliative care organisations, including Myton Hospice as well as other community and hospital palliative care teams
  • Own and manage the Palliative Care shared drive ensuring documents are updated and filed appropriately
  • Manage stationary orders
  • Support team with adhoc admin requests as needed
  • Liaise with ICT to facilitate access to relevant services and products
  • Assist in the production of the team newsletter
  • Manage emails sent to the team email address

End of Life Quality

  • Update End of Life Care Resource Packs (Treansform Boxes) on wards with ward clerks
  • Support CNS with resources to support end of life care such as comfort blankets hearts and relative bed

Clinical Lead and Lead Nurse Support

  • Support with diary management
  • Support with letters to team members
  • Support with clinics including booking, set up and letters
