Job summary

An exciting opportunity has arisen for Band 6 Neonatal Outreach Nurses to work within the Neonatal Community Outreach Service, hosted by UHCW. This model of Neonatal Community Care is provided across the Coventry and Warwickshire region, including University Hospitals Coventry and Warwickshire, George Eliot and South Warwickshire Foundation Trust.

The post holder will be an excellent team worker and will be responsible for ensuring the highest standard of clinical care and evidence based practice for infants within the scope of neonatal outreach, providing specialist on-going care for the neonate and advice, support and education to the family in the home environment.

The post holder will assist neonatal staff across the Local Maternity and Neonatal System (LMNS) at 3 sites in Coventry & Warwickshire (University Hospitals Coventry & Warwickshire NHS Trust (UHCW), George Eliot Hospital NHS Trust (GEH) and South Warwickshire NHS Foundation Trust (SWFT) with early and appropriate discharge planning in accordance with the criteria for community outreach care.

Main duties of the job

The Neonatal Community Outreach Team provides a service covering 7 days a week, from 08:00-18:00 Monday to Sunday.

Hours of work will be a mix of 08:00-16:00 or 10:00-18:00.

This service provides continuing community care for babies and their families in the home setting.

We are seeking highly motivated Neonatal Nurse's to join our forward thinking family focused neonatal team. The successful candidate will demonstrate the drive and motivation to support the implementation of such an exciting service across the sites.To be successful in this role you will need to be registered nurse/midwife with post specialist neonatal course experience.

Job responsibilities

UHCW has a level 3 Neonatal Unit with 8 intensive care, 8 high dependency and 20 special care and transitional cots. The unit is one of the designated network neonatal intensive care units for the West Midlands Neonatal Operational Delivery Network. The unit has achieved stage 3 Baby Friendly UNICEF accreditation and was rated as good in the 2018 CQC inspection of UHCW

The service will provide care from a cluster of sites including George Eliot and South Warwickshire Foundation Trust, both currently provide Special Care to babies.

1.Support the development, implementation and evaluation of the Neonatal Community Outreach Service.

2. Ensure a high quality, efficient and effective service, in support of an integrated LMNS approach.

3. To work within the hub and spoke model for the outreach team.

4. To carry continuing responsibility for a defined caseload, including the assessment of care needs and the development, implementation and evaluation of complex care programmes, ensuring continuous provision of integrated care across the LMNS in Coventry and Warwickshire.

5. To provide a seamless timely discharge from hospital to the home environment

6. Collect and collate data as required to support the outreach model

7. Review patient pathways and facilitate early discharge of patients

8. The post holder will take a proactive role in the education and training of the multi-disciplinary teams, providing on-going supervision as an expert in relation to neonates who require on- going care needs within neonatal community outreach.

9. Allocate, coordinate, monitor and assess outreach service workload, ensuring resources are deployed appropriately, in the absence of the Clinical Nurse Specialist.

10. Recognise and act as an advocate for patient, carer, service and organisation.

11. Accountable for own actions in accordance with Code of Professional Conduct. Will contribute to corporate objectives, acting within local, Trust and statutory guidelines and policies at all times, including all aspects of child protection; ensuring the safety of the baby at all times.

12. Ensure safeguarding concerns are appropriately managed, using local safeguarding procedures to ensure the safety of all infants within the Neonatal Community caseload.

13. Highly developed communication skills enabling maintenance of high quality service and ensuring timely management and resolution of unexpected events and situations.

14. A knowledgeable team member, supporting staff, service and policy development within Neonatal Outreach; a resource for staff, advising on national, local and trust policy and procedures/guidelines, ensuring that clinical governance is embedded in practice.

15. Support the Clinical Nurse Specialist in identifying and assessing complex clinical and non-clinical issues, implementing resolutions and monitoring/auditing outcomes. Will conduct regular audit to evaluate the service development.
