At UHCW, we are looking for a dynamic, passionate and motivated dietitian to join our growing service supporting primary care networks (PCNs) as part of the national additional role schemes.

The job is split between two roles. The first role will involve supporting a primary care network of 3 GP practices across Coventry in delivering care to their patients. The focus will be on diabetes and obesity but the case mix also includes cardiovascular disease, first-line gastroenterology and malnutrition. The second role will involve strategic leadership of the secondary care diabetes dietetic service ensuring up-to-date clinical guidelines and optimising patient pathways.

The successful post holder will be managed and supported by the dietetic team based at UHCW and work alongside and learn from existing post holders within other PCNs across the area.

Main duties of the job

As a service lead:

  • To provide service management support to the Diabetes Dietetic team.
  • To work with the relevant MDTs and team and service leads to manage and improve services.
  • Provide specialist dietetic advice on the dietary / nutritional management of adult patients within the service.
  • To work with senior colleagues in the dietetic department on department/Trust initiatives.

As a Primary Care Network Dietitian:

The post holder will work as a key part of the Primary Care Network (PCN) multi-disciplinary team, helping PCNs to strengthen community and personal resilience and reduce health inequalities.

  • To support partnership working with Coventry PCNs and University Hospital Coventry & Warwickshire Dietetic Department to deliver integrated care and reduce inequality
  • To provide specialist dietetic assessment, management and treatment of patients with Type 2 diabetes/obesity, cardiovascular disease, IBS (first line advice), gastro-oesophageal reflux disease, diverticular disease, malnutrition
  • To act as a source of nutritional expertise for PCNs in supporting this new service
