Job summary

We are looking for a Dietetic Assistant Practitioner to join our proactive Nutrition and Dietetic Team at UHCW NHS Trust.This post sits within the Oncology Team of 7 Dietitians and one other Dietetic Assistant Practitioner and is a full time (37.5 hours), permanent post based within the hospital.

The job will involve providing a professional and high standard dietetic service to a wide range of cancer patients, undergoing surgical and/or oncological treatments, including chemotherapy, radiotherapy and endoscopic interventions. You will see patients within the in- and outpatient setting (Arden Cancer Centre), under the guidance of the Macmillan Dietitians.

We have a large caseload of predominantly Head & Neck & UGI cancer patients on enteral tube feeding. This new post will focus mainly on supporting these patients to manage their tubes, nutrition and weight throughout their cancer journeys.

Main duties of the job

Your role within the team will include: review of nutritional care plans of patients on oral nutrition support and established enteral feeds under the supervision of Dietitians, ordering of nutritional supplies, maintaining databases and online patient management systems, facilitating patient discharges by providing equipment to wards and liaison with catering, telephoning patients at home to review their progress.

It will also involve carrying out/demonstrating basic tube feed care, bolus/flush training and trouble shooting with support from the Nutrition Clinical Nurse Specialists.

There will be opportunities to deliver education sessions to a range of staff on specific topics, with support from the Dietitians.Your role will also support the administration team with appropriate tasks relating to patient's nutritional care.We are looking for a highly motivated, practical individual with experience of, or an interest in, nutrition in healthcare. You will also have excellent communication and IT skills.

Main Duties/ Responsibilities

Clinical

  1. To work as part of a team, under the supervision of the Team Lead, supporting the provision of high-quality feeding practice and nutritional support to inpatients and outpatients and provide dietary education to patients.
  2. To provide basic tube feed care, bolus/flush training and troubleshooting with support from the Nutrition Clinical Nurse Specialists.
  3. To work under the supervision of Dietitians, to provide follow up for patients referred to Dietetics for dietary advice, and nutrition support, using behavioural management techniques to overcome barriers to change.
  4. To liaise with catering, nursing and dietetic staff to ensure the provision of a satisfactory meal service for in-patients.
  5. To assist in menu planning including carrying out dietary analysis as appropriate.
  6. To obtain patients, weight, height, calculate BMI and check Nutrition Risk Score (MUST) as appropriate.
  7. To measure patients anthropometry (i.e. Mid upper arm circumference, mid arm muscle circumference, hand grip dynamometry and bed scale weights) in selected patients to monitor nutritional status.
  8. To obtain patients biochemical results using the Clinical Result Reporting System (CRRS) as requested by Dietitians using defined criteria.
  9. To review patients assessed to be at low or moderate risk of malnutrition, who have been triaged by Dietitians.
  10. To encourage patients to eat and drink their prescribed nutrition to maximise their nutritional intake and status
  11. To liaise with Dietitians, ward staff and/or wider multi-disciplinary team regarding patients monitoring and progress.
  12. To review prescriptions of nutritional supplements and snacks within agreed boundaries.
  13. To actively participate in the nutrition review process for in-patients including patients on stable tube feeds.
  14. To arrange for supplements to be given to patients either on discharge from hospital or as

a sample to trial, as requested by Dietitian.

  1. To process communications with GP's including arranging supplements in the community as requested by the Dietitians, updating GPs regarding feeding tubes and tube removals.
  2. To process communication with Community Dietitians updating patients dietetic plans and progress.
  3. To deliver feeding products and equipment to wards for patients being discharged on artificial feeding as requested by Dietitians and community nutrition nurses and register patients on the internet-based database.
  4. To register patients on an internet-based database for enteral feed and ancillary deliveries.
  5. To keep databases up to date.
  6. To provide telephone follow-up for patients as selected by the Dietitians.
  7. To develop dietetic displays for in-patients and to raise the profile of the Dietetic Department as required.
