Job summary
To provide advice and support on all pharmaceutical care issues, with particular emphasis on palliative care services provided at the Myton Hospice sites in both Coventry and Warwick.
The post holder will work within the Pharmacy department at UHCW and also the wider Myton Hospices team to provide a cost effective clinical pharmacy service which is patient and medicines orientated through effective supply, prescription monitoring and prescribing which ensures all patients receive safe, effective and timely medicines therapy.
The post holder will provide advice and support on all pharmaceutical care issues to patients and staff at Myton Hospices including daily ward visits, inputting on appropriate guidelines, attending MDT ward rounds and working on governance related issues as appropriate to the needs of the service. The post holder will work together with the Macmillan Senior Palliative Care Pharmacist to provide cover for each other when needed
The post holder is a member of the Oncology/ Haematology and Palliative Care pharmacy team, and will support and work in this speciality on a regular basis.
Main duties of the job
- To provide a comprehensive clinical pharmacy service to the Myton Hospices and take responsibility for medicines management, medicines optimisation and pharmacy clinical governance at the Hospices and any works or projects associated with this.
- Work closely with the medical and nursing teams at the Myton hospices to improve the pharmaceutical care of patients requiring palliative and end-of-life care.
- Develop and review policies, procedures and guidelines relating to medicines usage.
- Conduct audits and perform research relating to the use of medicines within palliative care and end of life services.
- Lead on the provision of education to healthcare professionals at the Myton Hospices in the pharmaceutical aspects of palliative and end-of-life care and support the Pharmacy team at UHCW to improve knowledge of palliative care as a speciality.
- Support the Myton hospices with cost-effective prescribing and rational medicines use.
- Be prepared and able to travel to Coventry and Warwick Myton Hospices on a regular and on-going basis as part of the clinical aspect of this role.
- Support management of drug alerts and medicine recalls at the Myton Hospices.
- Undertake investigations and review medicine-related CAEs at the Myton Hospices.
- Participate in Medicines Management and Governance Meetings at the Myton Hospices.
- To operate as an authorised pharmacist within the Pharmacy Aseptic Unit.

- Check prescriptions for clarity, safety and efficacy on wards, in dispensaries and the aseptic unit and when necessary complete Medicines Reconciliation to facilitate this. Query with, and provide the prescriber with appropriate information to effect a change in the prescription when clinically required, to ensure patients safety and effective therapy.
- Provide information to nursing and medical staff on wards on any aspect of medicines policy ensuring safe practice. Provide information to patients on allocated wards or in dispensaries concerning their medication and answer any resulting questions or queries.
- Provide when necessary, through checking, dispensing or supervision of technicians, an efficient medication supply service from any of the pharmacy departments, including the aseptic unit.
- Ensure patients discharge medication is accurate and dispensed in advance of the patients discharge, by writing the patients medication on the GP discharge prescription and conveying to the GP all necessary information concerning the patient medication to ensure a safe transfer to primary care. Reduce pharmaceutical waste by utilising patients own drugs in the discharge process.
- Within your designated clinical area of responsibility, ensure best practice and clinical governance is applied to drug therapy.
- Provide clinical pharmacy and prescribing services within the Trust as an independent prescriber (as required).
- Support the Associate Director of Pharmacy - Cancer/ Technical Services/ Clinical Trials in achieving long-term objectives of the department and its strategic direction including devising and reviewing SOPs and guidelines as needed.
- To participate in the competency based assessments and training of junior staff within the team. To ensure areas of concern in the clinical and operational performance of staff within the team are identified and reported to the Deputy Director of Pharmacy. To participate in performance management of members of the team as needed.
- Ensure that appraisals and personal development reviews for allocated pharmacy staff are completed. Participate in personal review and working to achieve set objectives. Participate in recruitment of staff.
- Work flexibly across all of the Pharmacy departments, covering the work of colleagues during peak periods or absence.
- Participate in appropriate training and development activities including your own personal development and participate in the education and training of colleagues and all other healthcare professionals on medicine legislation, pharmacology, medication risk and best prescribing practice. Contribute to local undergraduate and postgraduate education and training within your speciality, both within the Trust and for other programmes of study. To work with the Lead Pharmacy Technician/Pharmacist for Education and Training to ensure the effective delivery, assessment and induction of staff in the department.
- Participate in weekend, Bank Holiday and on-call working as may be required.
- Undertake any other duties as required by the Director of Pharmacy in accordance with the grade and nature of the post across the sites of the Trust.
