University Hospital Plymouth NHS Trust is part of the South West Neuromuscular Operational Delivery Network (SWNODN), with the team based in Bristol and Plymouth. Significant experience of working with patients and their families, an ability to work across different organisations / MDT.

Excellent communication skills / caseload management skills.

A full valid driving licence with access to suitable transport for business use (alternative arrangements may be made for a disabled candidate)

Main duties of the job

You will work in partnership with other Specialist Neuromuscular Network colleagues, to contribute to the development of the service, and work in partnership with other service providers to maximise the health, independence and quality of life of people living with a neuromuscular condition.

With your experience of working with people with long term conditions, you should be able to assess complex situations and manage challenging relationships with empathy and sensitivity, whilst supporting patient needs, acting as an advocate and negotiating effectively. You will possess excellent communication skills and be able to connect effectively with people of all ages, cultures or religious backgrounds with dignity and respect.

You will provide ongoing information and support to people living with a neuromuscular condition and their families/carers. You will ensure patients can maximise their independence with the flexibility, sensitivity and forethought required to meet the specific physical, mental, emotional, spiritual, cultural and social needs of the individual.

***Preference will be given to internal Trust staff, as well as 'Priority' and 'At Risk' status including NHS At Risk staff throughout Devon who are able to display recent relevant experience as dictated by the Person Specification.***
