Job summary

Are you an existing junior sister/ charge nurse who is passionate about dialysis access? An exciting opportunity has arisen for an expert renal nurse to join the dialysis team, to work with the vascular access practitioner to lead and develop the dialysis access service. This is a permanent position for 37.5 hours per week.

You will be responsible for ensuring that CKD 5 patients who have opted for dialysis, have access in situ prior to commencing dialysis and monitoring of all access on patients on dialysis. The CKD team are based at the main site in Derriford whilst the main dialysis unit is situated approximately 1 mile away from the main Derriford site and we are located in Estover. There is very easy access to the unit and parking on unit's ground is free of charge. Applicants will be expected to travel to all satellite units in Holsworthy, Kingsbridge and Saltash.

Main duties of the job

  • The overall job purpose of the dialysis vascular clinical nurse specialist is to provide highly specialist dialysis access care within the hospital and associated satellite units to all renal patients, including those with complex access needs using advanced clinical knowledge that is evidence based.
  • Will be responsible for leading and coordinating the clinical, managerial and education requirements for dialysis access.
  • This will include acting as an expert resource for pre-dialysis clinical nurse specialist team and dialysis nursing team to support them in the delivery of effective care.
  • They will be responsible for ensuring a good working environment in which all patients and carers receive a high standard of care.
  • They will be responsible for implementation and monitoring of assessment tools to ensure optimum safety and care.
  • They will be expected to play a proactive role in quality and service improvement and working closely with the lead nurse and multidisciplinary team to ensure monitoring and auditing of clinic standards of care is undertaken.
  • Working with the Southwest renal network to ensure equality and access for all patients.

To regularly monitor activity and waiting times for vascular access against NICE guidelines, reporting to the lead dialysis nurse on monthly basis. To alert the lead dialysis nurse to potential problems before they arise.

Co-ordinate a renal access specialist pathway for pre-operative assessment of patients for dialysis vascular access, working in conjunction with pre-dialysis, surgical and nephrology teams and within agreed protocols.

Provide dialysis access clinic to provide post operative care and identify whether the access is working, identify complications, arrange duplex scans and re-listing of patients where necessary following agreed protocols.

Provide individualised information to the patient about the vascular access process.

Arrange timely pre-operative duplex vein mapping of patients needing access.

Review patients duplex scans and all relevant patient information in conjunction with a member of the surgical team formulate an individual and management care pathway.

Undertake arterial-venous (AV) fistula/graft flow measurements and re-circulation using a specialized transonic monitor in all haemodialysis patients, interprets results and refers on, if required to vascular scanning, interventional radiology, or vascular surgeons.

Identify complex cases and flag up to surgical and nephrology team.

Assist Consultants with CVC or Tenchkoff insertions.

Education

Attend relevant clinic and multi-professional meetings, seminars, and conferences.

Organise educational events to disseminate relevant quality service information and changes for venous catheter care.

Train identified nursing staff in the use of transonic monitoring to provide quality assurance for all vascular access patients.
