Within the Inflammatory Bowel Disease (IBD) service there is the opportunity to work as part of a dedicated MDT consisting of clinicians, nurse specialists and dietitians to deliver high quality care to a diverse patient population. Roles for an advanced specialist pharmacist would include, developing proactive and reactive therapeutic drug monitoring (TDM) services for patients on Biological agents and integrating with colleagues across the Devon ICS to share best practice and driving quality improvement within prescribing in IBD.

There is also scope to become a pharmacist prescriber within the Nutrition Team.

Depending on the stage of your career, further study will be encouraged and supported. Practice research is also encouraged in this role; we currently have Band 7 Academic intern pharmacists.

Main duties of the job

  • This post holder will be responsible for the provision and development of pharmaceutical care for patients within the areas of Gastroenterology & Nutrition.
  • Act as team leader for the ward-based pharmacy team and as team member of the Clinical Pharmacy Team.
  • Undertake teaching and clinical supervision of pharmacy staff and other healthcare employees.
  • Provide specialist clinical pharmacy services and advice, demonstrating a high level of clinical effectiveness.
  • Work closely with care group lead pharmacist & associate chief pharmacist clinical services on the strategic development of clinical pharmacy services, establishing clearly defined roles for pharmacist non-medical prescribers within Gastroenterology & Nutrition care areas.
  • Ensure efficient use of resources and systems including monitoring drug expenditure within the Gastroenterology & Nutrition clinical areas.
  • Work with the clinical and governance teams to develop guidelines, protocols and PGDs.
  • Undertake audits and engage with Quality Improvement and practice research
  • To provide clinical pharmacy support in co-ordinating the care of patients requiring parenteral nutrition across the Trust.
  • Ensure Gastroenterology patients are supplied medicines via the most cost clinically appropriate and cost effective route.
  • Undertake medication review to ensure that patients treatments are monitored in accordance with agreed best practice pathways
  • Deputise for the Lead Pharmacist Medicines Care Group as required

The Gastroenterology Department at UHP provides and comprehensive range of services to investigate and treat disorders of the digestive tract including abdominal pain, diarrhoea, indigestion, difficulties swallowing, Crohn's disease and colitis.

UHP provides a tertiary service for the investigation of oesophageal diseases, oesophageal EMR and RFA of early Barrett's cancer and surgical treatment of oesophageal, gastric, Hepatobiliary and pancreatic cancer.The Trust is designated as a type III intestinal failure service and has the capability to offer the type II services for patients across the Peninsula.

The Pharmacy team are considered an integral member of the multidisciplinary team supporting both Gastroenterology and Nutrition service, joining a team of clinicians, nurse specialists, dieticians and other allied healthcare professionals.

  • Responsible to the Lead Pharmacist Medicines Care Group and Associate Chief Pharmacist Clinical Services for strategic planning and delivery of clinical pharmacy services to patients under the care of the Gastroenterology & Nutrition Service Lines.
  • Support the Chief Pharmacist / Deputy Chief Pharmacist / Associate Chief Pharmacist and Care Group Lead Pharmacist in developing, monitoring, and reporting on drug expenditure and medicines budget and the delivery of financial savings as part of the pharmacy CIP.
