We are looking for an experienced doctor who is comfortable working within a strong multidisciplinary team to coordinate care for people with complex needs in a community hospital setting.
The right person for the job will have experience working with older/ people living with frailty, and ideally experience in community hospitals /or rehabilitation.
Our department has grown considerably since integration with our local acute hospital and we are proud to work together with our partners to provide community hospital care, acute hospital geriatric care, community geriatrics, neurological rehabilitation and medical education across a number of sites. Main duties of the job
- Assessment of all new admissions to the unit and timely completion of discharge documentation.
- Review of clinical deterioration in patients at the guidance of nursing staff or medical colleagues.
- You will be expected to supervise trainees in your clinical areas.
- The post holder will be expected to provide reciprocal cross cover for leave of other medical staff on the MGH site. Short-term sick leave will be similarly covered.
- Secretarial support is available from the Ward Clerk for inpatients.
- The post-holder will work as a member of the multi-disciplinary team, ensuring patient care is based on need with services that are accessible and acceptable to patients and carers. They will be available by appointment to meet with family members and other carers when appropriate to discuss patients' management and progress.
- Inclusion on the non-resident on call rota based at Mount Gould Hospital.
- The post-holder will be expected to uphold the principles of clinical governance and to adhere to the organisation's clinical governance requirements as outlined in Standards for Better Health. You are expected to be actively engaged in the organisation's clinical governance programme.
- Dedicated office space and a support from a shared secretary are integral to this post. IT facilities and support are standard, including access to unit/team performance data.

About us
Our Local Care Centre Team consists of a Specialist Grade doctor in geriatric medicine, two GPs with a specialist interest in older people, a consultant community geriatrician and a consultant geriatrician. We are also supported by our general manager for the community hospitals and ourService Line Director for Healthcare of the Elderly services.
We enjoy support on the Mount Gould site from four GPVTS trainees, one Foundation Year 2 trainee and one ST3+ in Geriatric Medicine. There are 6 nursing and medical teams across the two wards. You will be responsible for the medical care for your teams patients. You will cross cover absent colleagues too.
Our Main Service: Kingfisher & Skylark Wards, Local Care Centre, Mount Gould Hospital (UHP) There are now 88 beds across these two community hospital wards for geriatric and post-acute rehabilitation. The case mix is broad and includes those following orthopaedic surgery, trauma and amputation as well as acute physical illness in older people. The majority of patients are admitted from Derriford Hospital following an acute episode of care. There is an experienced multidisciplinary team across these wards including nursing, physiotherapy, occupational therapy and excellent psychiatric liaison nurse support.
Out of hours cover is provided through senior nurse on site, supported by off-site hospital-wide on-call junior doctor and senior on-call rota (in partnership with Livewell Southwest [LSW]).
