Job summary

We are seeking a dedicated and passionate Parkinson's Nurse Specialist to join our team. In this vital role, you will support the provision of specialist services to individuals living with Parkinson's Disease, both within the hospital setting and across the community. Your contributions will play a crucial part in improving the care and quality of life for patients and their carers.

You will take responsibility for the assessment, planning, and implementation of care, working closely with the Nurse Specialist and a team of healthcare professionals to deliver holistic care that improves patient outcomes and experiences. The role is designed for a proactive, experienced nurse who is committed to advancing Parkinson's care and working collaboratively with both internal and external healthcare teams.

As part of your role, you will support the ongoing education and training of colleagues, empowering them with the knowledge and skills to provide high-quality care. You will be expected to facilitate good communication and strong working relationships between various professionals, ensuring coordinated and seamless care for patients.

Main duties of the job

  • Provide expert assessment, planning, implementation, and evaluation of care and treatment for patients with Parkinson's Disease in collaboration with the Nurse Specialist and multidisciplinary team.

  • Deliver high-quality, evidence-based care to patients, ensuring their needs are met at every stage of their condition.

  • Support the ongoing development and training of multi-agency colleagues in the area of Parkinson's disease care to enhance the overall patient experience.

  • Work towards meeting both National and Local Health Agenda targets in line with the Local Delivery Plan.

  • Proactively contribute to the continuous improvement of services, ensuring they are patient-focused and cost-effective.

  • Foster strong, collaborative communication and relationships among all professionals involved in the care of patients with Parkinson's Disease.

  • Deliver a responsive service that meets the ever-evolving demands of Parkinson's Disease care, ensuring all patient needs are met promptly and efficiently.
