Job summary

We are looking for an energetic individual, who is ready for a new engagement role, or wanting to develop their current engagement skills to promote and support digital inclusion across Primary Care Networks in the Gedling Area (Byron, Synergy, Arrow Health and Arnold & Calverton PCNs). Supporting patients and primary care staff to understand, access and utilise different digital platforms to get the best from local health services such as the NSH App and Patients Know Best. The Digital inclusion co-ordinator will work across GP Practices, with the Borough Council, voluntary sector, and patient groups to reduce health inequalities by promoting digital inclusion.

Main duties of the job

Supporting the PCN to reduce health inequalities via digital means, increasing the uptake of digital offerings by supporting citizens and patients access primary care through digital technology.

The Digital and Social Inclusion Co-ordinator will provide day to day support to staff and citizens across the Primary Care Network.

This will include supporting and working on delivering and maintaining an overview of the projects schemes. Promoting the project's offerings through outreach work, finding innovative ways to connect with existing and potential stakeholders.

Acting as first point of contact for any programme and project enquiries. The Digital and Social Inclusion Co-ordinator will be a pivotal member of the team, providing key administration activities to support the effective delivery of the programme and projects initiatives.

About us

The role is being hosted by Nottinghamshire Health Informatics Service (NHIS), on behalf of the Primary Care Networks placed within Gedling (Byron, Synergy, Arrow Health and Arnold & Calverton PCNs).

Working with NHIS and the PCNs will provide a supportive and encouraging team environment, where you can develop and thrive to make a difference to patients, by supporting them to access healthcare services.
