The postholder will ensure that medications are used safely, effectively, and cost-effectively for all Trust patients. This is a rotational position, primarily based at the Denmark Hill facility.
The post holder will be managing a Medicines Management Pharmacy Assistant (MMPA) and if necessary, another junior staff.
Main duties of the job
- To provide a specialist patient-focused technical pharmacy service.
- To counsel patients and/or their carers on their medication.
- To assess suitability of patients' medicines on admission and discharge.
- To provide appropriate and sufficient supplies of medication during admission and on discharge.
- To facilitate discharge through the timely and efficient supply of medicines.
- To help with Control Drug returns
- To liaise with ward and pharmacy staff to facilitate the efficient transfer of medication and relevant information when patients move wards.
- To follow through any relevant queries arising from patient transfer or discharge.
- To liaise with pharmacists, ward staff, general practitioners, community pharmacists and other primary healthcare professionals as required to resolve queries and ensure accurate patient medication histories are obtained, in line with local policy.
- In liaison with pharmacists, to obtain and document patient medication histories, after appropriate training and/or accreditation.
- To support with individual patient medication management issues, within own skills and expertise.

- NVQ Level 3 plus accredited underpinning knowledge [BTEC in Pharmaceutical Science or equivalent] or equivalent.
- Registered as a pharmacy technician with the General Pharmaceutical Council (GPhC)
- Accredited Checking Pharmacy Technician (ACPT) qualification or working towards.
- Mandatory CPD to maintain fitness to practice
Desirable
- Successfully completion of development courses relevant to practice
- HEE Medicines Optimisation course or equivalent (or to be achieved within 6 months of employment)
