Job Title: Paediatric Specialty Doctor (employed as Havens Specialty Doctor)

The post will be based at the CYP Havens daytime service in Haven Camberwell (King's College Hospital site), where a refurbished unit provides paediatric medical examinations (after recent assault, but outside a forensic window) and follow-up for children disclosing (or where there are concerns about) sexual abuse. There will also be potential need to travel to the other two Haven sites - Haven Paddington (St Charles Hospital) and Haven Whitechapel (near the Royal London Hospital).

The Havens are keen to provide trainee paediatricians with a tailored, closely supervised training experience in the area of child and adolescent sexual assault/abuse. We are aware that there is a both a shortage of potential to obtain expertise during training in this highly specialised area and an impending shortage of suitably experienced paediatricians to carry out this work at a high standard. The training available during the 6 month period would meet the Level 3 learning outcomes of each learning domain as per the revised RCPCH Curriculum, in particular Safeguarding (Domain 9), Education and Training (Domain 10), Professional values and behaviours and professional knowledge (Domain 1), Professional Skills: Communication (Domain 2), Leadership and Team working (Domain 6), and Quality Improvement (Domain 8).

Main duties of the job

The post will also provide the opportunity to gain experience in clinical governance and management by attending and participating in monthly Havens Risk & Governance and Operational meetings. The paediatric specialty doctor will be encouraged to review proformas and guidelines and develop new ones where appropriate. The paediatric specialty doctor will complete an audit or quality improvement/research project and be involved in service and workforce development projects during the six-month post, with the aim of subsequent publication and/or presentation at a scientific meeting. Additional educational opportunities include Havens Educational, Research and Training meetings as well as pan-Havens Peer Review Meetings.

Forensic medical examinations are provided in the Havens' forensic suites. The paediatric specialty doctor will be able to shadow Havens paediatricians and Sexual Offenses Examiners (SOE) during forensic examinations. They will also have the option of gaining further experience of victims of sexual assault at all ages (the predominant age group is adolescent and young adult) by joining the Havens out-of-hours Forensic examination rota as an SOE. This would require attending a Havens course for Sexual Offences Examiners and a period of shadowing cases.

About us

The Havens are London's Sexual Assault Referral Centres (SARCs) based in Camberwell, Paddington and Whitechapel. We have a strong partnership with the Metropolitan Police Sexual Offences, Exploitation and Child Abuse Command. The Havens' Children and Young People's service (CYP Havens) supports children and adolescents when there are recent concerns about sexual assault/abuse. It provides expertise in holistic care of children and young people including assessment of injuries, forensic and medico-legal work, safeguarding, sexual health, counselling, psychology, advocacy and other specialist support.

There are additional services for adolescents in the main Haven service. The paediatric specialty doctor will be part of a multi-disciplinary team that includes Paediatricians, Sexual Offenses Examiners, Advocates, Psychologists, social workers and independent sexual violence advisors (ISVA's). Safeguarding is a priority and there are weekly multi disciplinary team meetings. The team liaises very closely with police and social services. There will be additional responsibility to work with the Family Nurse Advocate and Adolescent Nurse in relation to clinical oversight and participation in paediatric liaison with health, police and other agencies in relation to children who have been seen in the Havens.

The Havens provides acute and follow-up services to adult and child victims of rape and sexual assault who either self-refer or are referred via the police or other agencies.

We offer a 24/7 service providing telephone advice, forensic examination and immediate medical aftercare. We also provide follow-up medical care, Independent Sexual Violence Advocacy (ISVA) and psychological services, which may generally be accessed up to a year post-assault.

The Havens aims:

To provide excellent clinical care in to those who have been acutely sexually assaulted. This will be provided in conjunction with forensic medical examinations when appropriate which may contribute to the investigation of the allegation and assist the Criminal Justice Process;

To sustain and develop partnership working with the Metropolitan Police Service, along with healthcare staff both within KCH and other health care settings and the voluntary sector;

To develop and improve standards of care, with innovative training methods;

To utilise patient feedback, audit and research to maintain and improve standards; and

To contribute to the development of regional and national SARC policies and to other SARCS, where appropriate.

2. Clinical organisation.

The Havens is a multidisciplinary clinical service comprising medical, nursing, paediatric, crisis work, ISVA and counselling and psychology teams. The day time service is supported by an additional cohort of Havens-trained out-of-hours examiners, paediatricians and crisis workers to deliver the 24/7 forensic service.

The Havens sits within Planned Medicine at KCH. The Havens is led by the Clinical Lead for the Havens Adult Service, the Clinical Lead for the Havens Children and Young Peoples Service, Matron and Service Manager who are part of a wider senior leadership team which includes the leads for other disciplines within the service.

Departmental Staffing

The CYP Havens team comprises a Consultant Clinical Lead for CYP, 1.2 WTE substantive consultant paediatricians, 3 additional out of hours consultant paediatricians and 1 locum day-time consultant paediatrician, a dual-trained sexual offences examiner, a speciality post doctor, a Family Nurse Advocate, a Child and Family Clinical Advocate, an Adolescent Nurse Advocate, Child Psychologists, and Children and Young Peoples ISVAs.

Across the Havens three sites, we have:

Approximately 150 multidisciplinary staff

Approximately 9 WTE daytime forensic physician posts of whom 3 WTE are forensic nurse examiners and 6 WTE are sexual offences examiners (doctors)
