Job summary
The Critical Care Outreach Nurse in iMobile will provide holistic care that supports patients through their acute/critical care illness.
The post holder will have the knowledge and skills to assess and formulate management plans for critically ill patients as part of the multi-disciplinary team. The post holder will participate with the strategic development of the iMobile service within the organisation.
Main duties of the job
- To work in close co-operation with multi-disciplinary teams, facilitating high standards of health to patients who are critically ill outside the ICU setting.
- Assess the health needs of the deteriorating patient through physical examination, history taking and diagnostic studies in consultation with the iMobile consultant, SpR or SNPs / ACPs.
- Develop and manage a plan for the patient in consultation with the iMobile consultant
- Work closely with the Hospital at Night team (H@N), medical, nursing and physiotherapist teams on the wards.
- Provide management and leadership support for the iMobile team.
- Assist the iMobile nursing lead to achieve key performance indicators and success measures for the iMobile service.
- Actively be involved in audits and research as part of service development.
- Assists in the training of nurses in theoretical knowledge, practical skills and in expanding their roles.
