Job summary

Applications are invited from trainees with an interest in Intensive Care Medicine, Anaesthesia or Hepatology. These posts are generally suited to applicants who have already completed core training and who have embarked on speciality training. It is not unusual for us to have a number of post-CCT fellows on the rota. Intensive care experience and advanced airway skills are desirable but not essential. Membership/fellowship of the relevant Royal College or equivalent for overseas applicants is preferred. The post available is for 6 months, up to 2 years dependent on requirements/service requirements.

A brief/general overview of your role would include the following:

Handing over from night shift doctor to day shift occurs at 8.00am and is supervised by the consultant. Daily comprehensive morning ward rounds are supervised/undertaken by the consultant Intensivist. Procedures are normally undertaken in the afternoons. Ward rounds are multidisciplinary in nature incorporating medical, nursing, physiotherapy and pharmacy staff. Regular microbiology input is available as is dietetics. Every evening at 4 pm a multidisciplinary meeting is undertaken where all patients are discussed by the Intensivists, Hepatologists and Liver transplant/HPB surgeons. An evening ward round is then conducted, led by the ICU consultant along with the SCF and senior nurse in charge alongside the night SCF.

Main duties of the job

The Liver Intensive Care Unit at King's College Hospital has a single vacancy on the Clinical Fellow rota commencing August 2022.

The Liver Intensive Care is a unique facility with a worldwide reputation for the care of critically ill patients with liver disease. Staffed by a specialist team of intensive care doctors, it has been setting international standards for patient care for more than two decades and is a leading centre for the development and use of advanced and innovative therapies.

We have a special interest in the management of acute liver failure, complications of portal hypertension and chronic liver disease and liver transplantation. We also admit patients from other disciplines and have a pedigree in the management of complex haematology, cardiology and cardiothoracic surgical patients requiring circulatory support (including ECMO) and general medical and surgical patients.

Within both the Institute of Liver Studies and the Trust (organised by the Department of Postgraduate Medical and Dental Education) there is a comprehensive teaching programme with protected teaching time and access to the excellent medical school facilities.

The post includes on call duties.

Successful candidates will be involved in fixed clinical commitments which would be determined by both their clinical training needs and the service requirements of the Liver Intensive Care Unit. The post holder will be expected to participate in the on-call rota on a flexible basis in accordance with the European Working time directive, and local trust policy. The doctor undertaking the fellowship will be scheduled to undertake non-clinical, management and research aspects of the post through dedicated rostering. There will be an expectation that the doctor will also be responsible for departmental Journal club organization and scheduling.

There may be an opportunity to extend the attachment to undertake a formal postgraduate research degree, but this would require appropriate agreement and assessment.

In following the objectives of the New Deal agreement, hours of duty and rota arrangements may change before and during the period of employment. Acceptance of an offer of this post will include acceptance of the changes including remuneration in accordance with the Provisions of the agreement and circular (MD 1/92). All incumbents will be fully involved in discussions on planned changes.

The Post holder will accept that they may be required to undertake duties during occasional emergencies at the request of the appropriate consultant in discussion, where practical, with 4 both senior and junior colleagues. The post holder would not be expected to undertake work of this kind for long periods of time.

Medical clearance, and in some circumstances, examination is required prior to appointment.
