Job summary

We are seeking to recruit an ST4-9 (Clinical Fellow) in Paediatric Respiratory Medicine, to work in the Paediatric Respiratory and Cystic Fibrosis Centre at King's College Hospital. King's College Hospital is a major provider of tertiary paediatric respiratory services, houses major Paediatric and Adult Regional Centres for Cystic Fibrosis and is an accredited centre (UK and Europe) for training in paediatric respiratory medicine. It provides both secondary and tertiary care mainly though not exclusively for children in South East London, Kent and East Sussex. The multidisciplinary team is led by Dr Atul Gupta and five other paediatric respiratory consultants. This post offers the opportunity to attain middle grade experience of a National Grid trainee in paediatric respiratory medicine at an accredited London training centre.

General information

This post is suitable for paediatric doctors with middle grade experience who have evidence of achieving at least ST4 competencies in paediatrics at the time of the appointment, some experience in paediatric respiratory medicine and eligibility to work in the UK. Full MRCPCH is highly desirable.

Main duties of the job

At King's, children with medical respiratory problems are admitted to Toni and Guy Ward and those with surgical problems to the Children's Surgical Ward. Children requiring high dependency or intensive care are admitted to the Thomas Cook Critical Care Centre in the Variety Children's Hospital where the respiratory service provides regular support and advice for children receiving invasive and non-invasive ventilation. Approximately 300 children with asthma and related wheezing illness are admitted to King's each year. Many children admitted acutely via the ED with asthma or other serious respiratory illnesses are referred to the respiratory service. We provide an acute tertiary referral service to other DGHs in our region and beyond for the referral of complex respiratory cases including paediatric empyema, pneumothorax, necrotising pneumonia, with back-up from paediatric or cardio-thoracic surgery.

There is a paediatric tuberculosis clinic and a weekly Difficult Asthma clinic conducted by a consultant and CNS, as well as a weekly nurse-led asthma clinic. Monthly multi-disciplinary non-CF bronchiectasis and ventilation (NIV and invasive) clinics are also held. All consultants cover each other for clinics when on service or away on annual or professional leave. The CF multi-disciplinary team are involved with a weekly cystic fibrosis clinic, weekly CF annual review clinics, CF baby clinics, CFSPID clinics and ad hoc CF reviews throughout the week. In addition, we do joint CF outreach clinics throughout the Southeast region.

The paediatric cystic fibrosis service at Kings has been established for over 30 years. It is an accredited Regional Centre at the hub of a mature clinical network, which extends through South London, Kent and East Sussex. The centre cares for approximately 200 children. Joint outreach clinics are conducted in 7 paediatric units in the region with the full multi-disciplinary team. Children from anywhere in the region, however, may be reviewed at or admitted to Kings between outreach clinics depending on clinical need and a number of children receive full CF care at Kings because they live locally or have particularly severe or complex problems. Transition to the adult service is well-established and includes joint transition clinics with the adult CF multi-disciplinary team which has the great advantage of being on the same site. From its inception, we have been a member of the London CF collaborative, which has produced seminal reports on early lung function in CF infants. We are a CTAP (Clinical Trials Accelerator Platform) centre which has formed a partnership with other London CF centres for children on invasive and non-invasive ventilation.
