The successful applicant is expected to be available to attend a 1 day Intermediate Communication Workshop on Wednesday 3rd July 2024, delivered at The Clatterbridge Cancer Centre - Wirral.

We have an exciting opportunity to join our team of Haematology Clinical Nurse Specialists (CNS) as a Haematalogy Cancer Support Worker. The Band 4 Cancer Support Worker (CSW) role is central to the delivery of excellent patient care.

The Cancer Support Worker role is an integral part of the Haematology Team, who assist, support and co-ordinate the cancer pathway for patients. A vital aspect of the role is providing ongoing support for the patient and their families from the point of diagnosis, and supporting the consultants, cancer nurse specialists and other staff involved in the cancer pathway. The successful applicant will efficiently liaise and co-ordinate with the patient, relatives and all members of the multidisciplinary team, ensuring robust and effective communication.

Main duties of the job

To meet with patients at the point of diagnosis and provide ongoing support throughout their cancer treatment, signposting to relevant support services available.

Provide written information and contact numbers at the point of diagnosis.

The Cancer Support Worker will independently lead and manage end of treatment clinics, which provides support and guidance for patient's at the end of their treatment journey.

Develop and maintain effective working relationships with clinicians, managers and others within department and Trust wide service users.

Be the point of contact for all routine enquires facilitating communication and information flows, initiating and responding to correspondence and resolving enquiries

Coordinate the necessary assessments, appointments, or investigations to fast-track people back into the system if required.

Liaise with GPs/cancer services as appropriate.

Liaise with patients and other consultants' secretaries.

Using good communication skills, and appropriate tools and procedures, liaising as appropriate with the CNS team, when non routine, and refer complex decisions to the team for assessment and review.

Provide basic telephone advice and refer on or sign-post to other sources of support

Liaise closely with the MDT co-ordinator ensuring robust systems of notification and where appropriate co-ordinate transfer of care.

About us

The Countess of Chester Hospital NHS Foundation Trust provides services to West Cheshire and to Welsh patients covered by Betsi Cadwaladr University Health Board. The Trust works collaboratively within the wider Cheshire and Merseyside Health and Care Partnership. Its services are provided from three locations:

  • The Countess of Chester Hospital: providing 438 general and acute beds.
  • Ellesmere Port Hospital: providing 56 beds as a rehabilitation, intermediate and outpatient facility.
  • Tarporley War Memorial Hospital: a base for community services which serve the local rural population.

The Trust employs over 6258 staff (headcount) which includes temporary bank staff and provides acute emergency and elective services, primary care direct access services and obstetric services to a population of approximately 407,000. This includes 357,000 residents in Chester and West Cheshire which includes Ellesmere Port and Neston as well as the Deeside area of Flintshire which has a population of approximately 50,000.

The Trust is a busy district general hospital and in 2022/2023, there were more than 503,342 patient attendances (inpatient, outpatient and diagnostic) ranging from a simple outpatient appointment to major cancer surgery. This is an increase of nearly 10,000 compared to the previous year when there were 494,387 patient attendances.
