An exciting opportunity has arisen for a Senior Transfer Of Care Hub Administrator in the friendly and supportive admin team, within the Transfer of Care Hub.
We are looking for an individual who is an excellent communicator, is organised and confident using a variety of IT systems. The post holder will need a to Support the team with data reporting, supporting on key initiatives and projects within the team whilst supporting the development and skills of the Performance Support Assistant's.
Hours per week: 30
Main duties of the job
The successful candidate will provide essential support to a group of nurses, therapists, social workers and other professionals by communicating with a variety of services, both internally and outside agencies. They will be responsible for collecting, collating and reporting a variety of data to support the organisation.
