Are you a Healthcare Assistant who wants to be part of an innovative and dedicated Out of Hours team?

Do you want to support the community of Eastern Cheshire by providing excellent care at the patient's own home as part of a dedicated Crisis Response Service?

We are looking for an enthusiastic and committed Health Care Assistant to join our friendly and supportive Community Nursing Out of Hours team. The successful applicant will need to be a team player with excellent communication skills and a drive to provide high quality care. We actively encourage personal development and offer opportunity to complete in house training to develop the role of Community health care assistant within a domiciliary environment. The post holder will be expected to work flexibly to include weekends and bank holidays.

The post is for 19 hours per week to be worked as 3 x 1800-2200 and 1 x 1830-2200 evening shifts each week. Occasionally you may be required to cover a night shift 2200 - 0800 hrs. Pay will include some evening or night enhancements.

Hours per week: 19 (6 months fixed term)

Main duties of the job

To contribute and support the delivery of high standard of effective nursing care, as part of a nursing team, in support of and under the direction of a registered professional

To carry out assigned tasks, including direct nursing care and administrative duties as delegated by the nurse in charge

You will ensure that patients receive safe, individualised and effective care which supports health & wellbeing and promotes self- care where appropriate and possible.
