Job summary

An exciting opportunity has arisen to join in our new Urgent Crisis Response and Virtual Wards Team based in Macclesfield. We are a friendly multidisciplinary team made up of therapists, nurses, ACP's, rehabilitation assistants, and patient navigators. We also have a very close working relationship with Cheshire East Council, to enable us to support our patients at home, by accessing rapid care. We provide assessment and treatments for a wide variety of patients, with complex needs, across the East Cheshire locality, with the aim of preventing unnecessary hospital admissions and supporting people within their own homes..

We are looking for an experienced therapist/s, from either an Occupational Therapy or Physiotherapy background, with advanced clinical skills in Urgent Crisis Response and proven leadership skills in the community setting.

The Urgent Crisis Response service is a 7 day service operating between the hours of 8am-8pm. There will be a requirement for the successful candidate/s to work weekends, on a rota basis.

Hours per week: 37.5

Main duties of the job

The main duties of the Advanced Clinical Specialist Therapist with Urgent Crisis Response and Virtual Wards are:

  • To autonomously mange a defined complex patient caseload within a variety of different settings
  • Offer clinical leadership to the team by providing clinical supervision to junior members of staff and to be forward thinking and innovative in your approach to service development
  • To plan and deliver informal training to other health care professional staff groups, patients and carers
  • To continue to implement competency based training to develop clinical skills within the team
  • To work alongside the UCR & VW Team Leader to undertake clinical audit and quality improvement projects.
