Job summary

The Specialist Palliative Care Service are looking to recruit an experienced Band 8a MacMillan Specialist Palliative Care Team Leader to support and lead the existing team working in the East Cheshire locality, based in Macclesfield District General Hospital.

The Specialist Palliative Care Team Leader role is 60% clinical (patient facing) and 40% leadership and will work collaboratively with the Care Community Coach and Specialist Palliative Care Consultants.

The successful candidate will provide clinical leadership and expert advice to members of all professions involved in the care of palliative patients and will lead an Integrated Palliative Care Service within the Acute and Integrated Community Care Directorate.

The post holder will act as a specialist resource within the community and inpatient services within the local Acute Trust. Expertise in Specialist Palliative skills and knowledge will be required to manage comprehensive unique challenges faced by the service, maintaining a high quality of care for palliative and end-of-life patients.

The candidate must possess clinical assessment skills and knowledge with a non-medical prescriber qualification (or currently working towards completion).

Main duties of the job

  1. To collaborate with the Care Community Coach to empower the specialist palliative care team in the delivery of local and strategic goals of specialist palliative care patients to deliver outstanding person-centred care and providing assurance to the Acute and Integrated Care Community Directorate.
  2. To provide clinical leadership for the specialist palliative care team, acting as a point of contact for clinical advice and expertise on all matters relating to the specialist palliative care in collaboration with the Specialist Palliative Care Consultants
  3. The post holder will be required to have advance in communication skills to effectively deliver highly sensitive, complex, and sometimes contentious information to patients, families, and carers about the patient's end-of-life care plan.
