Job summary

East Cheshire Trust is looking to appoint a community coach in Macclesfield who will work as a change agent to transform and develop community services in 1 of our 5 community care teams. We are looking for someone who is willing to embrace the challenge of integrated working across health and social care, creating and delivering services which our local population and staff can be proud of.

In return, we are looking for someone who is committed to ensuring that all services within the locality deliver community services are delivered in an effective, efficient and safe manner.

Main duties of the job

Alongside the Clinical Lead for the care community, you will be the driving force to inspire your teams to take up the challenge of introducing new ways of working, always focused on our patients and the population as a whole. You will provide strong and effective leadership and line management to the teams within the Macclesfield care community, working alongside Community Coach colleagues to strengthen integration of services and locality working relationships. This will include proactive engagement and involvement of local health and social care commissioners, providers, patients & the public, third sector and all other stakeholders.
