Job summary

We have an opportunity for an experienced Band 5 or a band 6 Occupational Therapist to join our established community therapy team covering the CHOC care community, providing healthcare to the local population of Congleton and Holmes Chapel.

The role will involve providing comprehensive assessment and treatment of adults within the CHOC care community. This may be within patient's homes, nursing and residential homes and within our rehabilitation beds.

You will be part of a wider integrated team consisting of GPs, district nurses, ACPs, Social workers, Physiotherapists and Therapy assistants.

An element of the role currently involves Crisis response, which is our 2 hour response service aimed at keeping people safe within the community.

The community therapy team currently works over 7 days, with staff covering evening and weekend shifts on a pro rata rota basis.

Developmental Band 5 roles will be considered for this post, and the successful applicant will be supported through competencies towards a Band 6 role within the team.

The community therapy service is a 7 day service from 8am-8pm. Successful applicants will be expected to work on a rota to cover the service for pro rata evenings and weekends.

However, the crisis element is currently under review and this part of the role may be moving to a separate centralised team and would then not be part of this role and therefore you would only be required to work weekdays. We will hopefully be able to update you at interview.

Main duties of the job

To act as a clinical specialist providing a high standard of comprehensive Occupational Therapy assessment and treatment service to patients with diverse health needs across Congleton and Holmes Chapel within Eastern Cheshire.

This care will be delivered in a variety of community settings, working as an autonomous clinician, using accurate analytical skills and clinical reasoning to interpret assessment findings, making diagnoses where appropriate, identifying patient problems, setting goals, and providing an appropriate treatment plan and progression.

The role will also involve acting as a rapid responder as part of the crisis response service across the East Cheshire locality. The Crisis response service is currently operational 7 days per week from 8am-8pm, but this element of the role is currently under review and may be move to a separate centralised team in the near future, we will be able to update you at interview

To utilise sound communication via liaison and referral to other agencies as required embracing care community team philosophies.
