Job summary

An exciting opportunity has arisen due to maternity leave to cover a first contact practitioner role located with Middlewood Primary Care Network.

Middlewood Primary Care Network (Bollington, Disley and Poynton GP sites) are working with East Cheshire NHS Trust to provide a FCP.

This is currently delivered by two part time FCP, one of which will be vacant due to maternity leave from November 2022 for 12 months.

As the FCP you will be expected to provide clinical expertise, seeing patients, without prior contact with their GP, to establish a rapid and accurate diagnosis and management plan.

The role will deliver FCP clinics across Bollington, Disley and Poynton.

You will be supported both by the trust and the PCN to develop and will be expected to work collaboratively with the other FCP and the Middlewood PCN team to continually improve patient care and pathways.

This post is fixed term for 12 months

Hours per week: 18.75hrs - part time

Main duties of the job

This role will be as a first contact practitioner, but with link to work closely with the current Musculoskeletal Physiotherapy team and orthopaedic service based at Macclesfield District General Hospital. The post holder will work in an integrated way with the primary care teams within Primary care networks, delivering first contact Physiotherapy whilst developing advanced clinical specialist skills to support advanced practice within this area, outpatient therapies and orthopaedic clinics. The role will require working closely within the primary care team and the orthopaedic team to develop strong links and pathways within the services.
