East Cheshire NHS Trust have several exciting opportunities available for suitably qualified therapists to become part of our established therapy teams within the community. We welcome applications from physiotherapists with a keen interest in working within an integrated community team, with proven experience in crisis response, in patient rehabilitation beds and general community rehabilitation.
This post is based at Congleton War Memorial Hospital within the CHOC Care Community Team covering the Congleton and Holmes Chapel area.
The post also includes running the pulmonary rehabilitation group for 1 afternoon per week. Experience in this area would be desirable, however training can be provided if not.
The successful applicant will be required to work in a shift pattern over a 7 day period, from 8am-8pm.
You will be required to be flexible at times and work across sites in East Cheshire to assist colleagues from other teams. It is therefore necessary to travel efficiently between sites and to rural areas.
Band 5 applicants with relevant experience are welcomed.
Hours per Week: Full and Part time hours will be considered.
Main duties of the job
To act as a clinical specialist providing a high standard of comprehensive Physiotherapy assessment and treatment service to patients with diverse health needs across Congleton and Holmes Chapel within Eastern Cheshire. This care will be delivered in a variety of community settings and includes delivery of Pulmonary Rehabilitation. The role will be working as an autonomous clinician, using accurate analytical skills and clinical reasoning to interpret assessment findings, making diagnoses where appropriate, identifying patient problems, setting goals, and providing an appropriate treatment plan and progression. The role will also involve acting as a rapid responder as part of the crisis response service across the East Cheshire locality. The Crisis response service is currently operational 7 days per week from 8am-8pm.
To utilise sound communication via liaison and referral to other agencies as required embracing care community team philosophies.
