Job summary

An exciting opportunity has arisen to have a band 6 Acute Hospital Learning Disability Liaison Nurse in our trust to further enhance our existing service within the acute care setting in both the emergency and planned pathways.

We are looking for a registered nurse with excellent communication skills and a 'can do' attitude, who is committed to improving the experience of patients with a learning disability in our hospitals.

This role will support the implementation and embedding of the NHSI LD Improvement Standards and enhance the smooth transition of individual's transition from home to hospital, support safe and effective discharge, incorporating reasonable adjustments and promotion of hospital passports.

You will be working very closely with the Lead Nurse- Vulnerabilities, the Learning Disability and Complex Transition Specialist Nurse, the Named Nurse for MCA/DOLS and other members of the Safeguarding team. The role will also include the post holder having an effective liaison with the Community Learning Disability Teams.

The purpose of this role is to ensure that adults with learning disability or autism achieve the best possible outcomes when accessing healthcare services.

Main duties of the job

In partnership with the multidisciplinary team, the post holder will provide a seamless specialist service ensuring that patients with a learning disability, autism and complex needs receive the highest standardof clinical care. They will provide specialist advice, education and support to health care professionals, patients and carers.

The post holder will hold a caseload of current inpatients and outpatients awaiting admission, treatment and care, liaising with patients, family, carers, and health professionals, co-ordinating and planning holistic person-centred treatment and care.

The post holder will ensure the safety and well-being of patients with learning disabilities and will work in a manner that promotes dignity and human rights through the adoption of person-centeredprinciples.
