The purpose of this role is to actively coordinate and facilitate prompt investigations, diagnostics and clinical review, collaborating with the multidisciplinary team, thereby improving quality of care and experience for patients ensuring timely access to haematology treatment. The post holder will be responsible for the creation and maintenance of a database to ensure that patients who are entered onto the patient stratified follow up pathway are appropriately managed.
We are looking for a proactive, creative thinker who enjoys problem solving to join our Navigator team. This is a varied role that brings great job satisfaction. This position is 37.5 hours per week Monday- Friday.
Main duties of the job
The post holder will work closely with the multi-disciplinary teams to ensure the efficient management of the patient stratified follow up pathway. To assist in the site specific data collection processes, diagnostic cancer pathway waiting times and national cancer data sets and input on various information systems.

- Being responsible and accountable for their practice and behaviour under the guidance and supervision of a registered practitioner and by working as part of the oncology multidisciplinary team.
- Working with the haematology team to provide coordination of high quality patient care through on-going telephone, face to face assessment and proactive identification for needs using basic knowledge, approved tools and procedures.
- Collaboration and communicating with the haematology team and the wider health care teams to ensure the provision of appropriate services are available to the patient and the right time.
- Acting as a single point of contact for patients, carers and the healthcare team. To triage enquiries and referring to the appropriate hospital department or community setting.
- Early detection of problems and putting in place action plans to reduce unnecessary in-patient admissions
- Documenting and monitoring all aspects of patient care coordination of service delivery.
- Support the distribution of any patient experience surveys
- Triage incoming calls and initiate appropriate responses according to assessment tools, protocols, individual pathways and by liaising with the haematology team
- Make pre planned outbound telephone calls to patients to assess their needs and monitor their progress. To then liaise and update the oncology team as appropriate

- Act as the patient advocate and facilitator in order to resolve issues that may be perceived as barriers to care.
- Contribute and coordinate the holistic needs assessment and the development of a care plan for patients with non-complex needs and monitor and review this as required with the patient.
- Evaluate outcomes of care delivery with the registered practitioner
- To coach patients and carers to understand the signs, symptoms or situations to be aware of that would indicate concerns to raise with a healthcare professional
- Inform patients and carers on how to make contact with the relevant services when they feel their condition or needs have changes, including what to do out of hours
- Effectively utilise a computer and various software applications in order to enter and maintain patient data, communicate with the multidisciplinary team and complete other IT tasks required by the post
